Introduction. A decreased serum high density lipoprotein-cholesterol (HDL-C) is a strong predictor of cardiovascular risk. However, total HDL is a very dynamic, changeable fraction, and does not perform the function of atherosclerosis markers. In the presented study, the pattern of serum lipids, including HDL-C subclasses (HDL2- and HDL3-cholesterol), in a middleaged Polish Lower Silesia population was defined. Materials and method. A group of 746 males and 1,298 females, aged 35–70, were investigated. All subjects were participants in the PURE study. Mean serum lipid levels were determined for groups selected on the basis of gender, age, cigarette smoking, drinking alcohol and place of residence (urban/rural area). The data were analyzed using STATISTICA 6.0 PL. Results. In multiple linear regression models, age was the most important independent and consistent predictor of total cholesterol (TC) and LDL cholesterol (LDL-C). The prevalence of low HDL-C (threshold 40 mg/dL in males, 50 mg/dL in females) was 16.5% for males and 22.6% for females. This gender-conditioned difference in the prevalence of low HDL-C was greater in rural (20.0% vs. 30.9%, respectively, in males and females) in comparison to urban (14.4% vs. 17.1%) areas. The lipid pattern was significantly worse in rural than in urban females. Female rural inhabitants showed higher triglycerides (TG) and lower HDL cholesterol (total and contained in subclasses HDL2 and HDL3). Simultaneously, a higher BMI, higher percent of smokers and drinkers and lower age of smoking female rural inhabitants in comparison to urban females were estimated. In the total population, cigarette smoking or drinking alcohol were associated with significant increases in TC, LDL-C and TG, also with decreased HDL-C (smoking) or HDL2-C (drinking). Two-way analysis of variance showed the existence of interaction between these risk factors in their influence on HDL-C and HDL3-C. Conclusion. In the middle-aged population of the Lower Silesian region in Poland the place of residence (urban/rural area) had a significant impact on the lipid pattern. This pattern is more atherogenic in rural women than in urban women.

Introduction. Prenatal physical activity has been increasingly recommended in recent years as the fundamental condition of physiological pregnancy and birth by health promoting organizations throughout the world. Objective. To determine the influence of prenatal physical activity on the course of labour and delivery. The practical purpose was to present prenatal physical activity as an effective tool in the implementation of the new Polish standard for perinatal care. Brief description of the state of knowledge. Reviewed publications report either a positive impact or no impact of physical activity on selected parameters of labour and delivery. The most frequently cited benefits of physical exercise during pregnancy include: shorter delivery, less frequent need for anesthesia, reduced risk of operative births, a lower rate of induction of labor, amniotomy, episiotomy and perineum lacerations, and improved neonatal outcome. Conclusions. A review of the literature shows that regular prenatal physical activity can help reduce medical interventions during labour, without having negative consequences for either the mother or the foetus. It should be an important tool to implement the Polish standard for perinatal care. There is a need to promote regular prenatal physical activity among women, medical personnel, and physical education staff. Detailed instructions for designing prenatal exercise programmes should be included in the new guidelines for physical activity during pregnancy, both in Poland and abroad. To support or negate the hypothesis of the positive effects of physical activity on the course of labour and delivery, well-designed research trials should be conducted with the properly structured prenatal exercise programmes in the intervention groups.

Introduction. The results of breast cancer treatment depend mainly on better detection in mammography screening and, consequently, a higher proportion of women with early stage of the disease. They depend also on a better access to health care services and the effectiveness of oncological treatment. One of the methods of breast cancer control is a breastconserving surgery. With a proper patients’ classification for the treatment, the results of the breast-conserving surgery do not differ in relation to mastectomy. That’s why, the availability of breast-conserving surgical methods is particularly important, especially in a population in which a population screening is conducted. Objective. The analysis of the selected aspects of the breast cancer treatment’s standard in Podlaskie Voivodeship. Patients and methods. In years 2001–2002, 709 cases of women with breast cancer were reported to the Voivodeship Cancer Registry in Bialystok. 659 women were diagnosed with a primary invasive breast cancer. Based on a cohort of 499 women treated with radical methods the following indicators, recommended by WHO for the evaluation of the breastconserving surgery’s availability, were calculated: a percentage of patients with whom breast-conserving surgery was used, a proportion of breast cancer cases receiving post-operative breast radiotherapy after breast-conserving surgery and a proportion of breast-conserving surgery in pT1 cases. Results. The breast-conserving surgery has been used much less frequently in Podlaskie Voivodeship than in Western Europe, but more frequently than in Poland in general. Conclusion. It should be aimed to provide a surgical treatment with the use of breast-conserving surgical methods to the highest possible percentage of patients. The increase of the percentage of patients treated with breast-conserving surgery methods can be a result of an effective screening realisation.

Opis:
Ionizing radiation affects the expression of adhesive and co-stimulatory molecules in lymphocytes. The physiological function of cellular isoform of prion protein (PrPc) is little known. Evidences indicate a link between lymphocytes activation and PrPc expression on their surface; however, no direct effect of radiation on PrPc level in these cells was investigated. The objective of this study was to determinate the effect of low doses of ionizing radiation on the expression of PrPc on the surface peripheral blood lymphocytes in the women operating X-ray equipment. In 36 female workers and 30 persons of the control group the PrPc expression on CD3 (T lymphocytes), CD4 (T helper), CD8 (T cytotoxic) and CD19 (B lymphocytes), as well as the percentage of lymphocytes with PrPc on their surface, were tested. Subgroups with respect to age and length of employment were selected. A signifi cant increase was observed in PrPc expression on CD3 and CD4 with lowered PrPc level on CD8 and percentage of CD8 cells with PrPc in workers compared to control. The PrPc level did not show signifi cant changes in subgroups in relation to age (below and over 40 years old) both in the investigated and control groups, whereas a lower percentage of PrPc expressing CD19 cells showed in employed women below 40 years of age. A signifi cant decrease was found in PrPc expression on the surface of CD3, CD4 and CD8 cells in the subgroup employed for over 10 years than in the subgroup with less than 10 years of employment.

Introduction. The biological activity of VEGF depends on the presence of its specific receptors on the endothelial surface: VEGFR-1, VEGFR-2, and on their soluble forms sVEGFR-1 and sVEGFR-2. The binding of the membrane-bound receptors with VEGF affects the permeability, proliferation and migration of vascular endothelial cells. This creates the necessary conditions for the vascularisation of solid tumours and for the spread of remote metastases. The sVEGFR-1 and sVEGFR-2 receptors are believed to be natural inhibitors of VEGF. Objective. To determine the clinical usefulness of VEGF and the sVEGFR-1 and sVEGFR-2 receptors level assay in women with primary breast cancer. The assessment also took into account: patient’s age, stage of the disease, histological grade, status of the axillary lymph nodes and size of the primary tumour. Material and methods. The concentrations of VEGF, sVEGFR-1 and sVEGFR-2 were ascertained in 103 women with primary breast cancer. The concentrations of VEGF in the plasma, and those of the soluble receptors sVEGFR-1 and sVEGFR-2 in the serum, were assessed by ELISA, R&D Systems. Results. The study found significantly raised concentrations of VEGF, sVEGFR-1 and sVEGFR-2 in the serum of women with breast cancer, relative to the values obtained from the control group. It was found that with increasing clinical stages of the disease, the levels of VEGF and concentrations of sVEGFR-1 and sVEGFR-2 also increased. Similar findings were noted when assessing the degree of the histological grade of the tumours. Significantly higher values of VEGF protein and the assessed receptors were obtained from women with metastases to the axillary lymph nodes. A positive relationship, though without statistical significance, was noted between the concentration of sVEGFR-2 and the size of the tumour. Conclusions. The high concentrations of the VEGF cytokine and the sVEGFR-1 and sVEGFR-2 receptors in women with breast cancer are responsible for giving rise to the processes of tumour angiogenesis. The concentrations of the VEGF protein and the soluble forms of the receptors sVEGFR-1 and sVEGFR-2 in the serum of breast cancer patients showed positive correlations with the clinical stage of the disease. These results point to the usefulness of VEGF assessment and its soluble receptors in the clinical evaluation of patients with breast cancer.
