Opis:
High-dose chemotherapy currently used in the treatment of children with cancer may induce late effects on psychosocial and cognitive functioning in some of them. The aim of the study was to evaluate psychosocial functioning of childhood cancer survivors living in rural regions. In total, 29 children entered the study. The children were diagnosed as having cancer between 1993-1995 in a single centre. Each patient was examined 5 years after the completion of cancer therapy. The children had been diagnosed with leukemia and lymphoma (72.4%), and solid tumours. Self-report questionnaires, as well as standardized psychological test (WISC-R, WAIS-R-PL) were performed for the evaluation of psychosocial and cognitive functioning of the patients. Most of the studied cancer survivors living in rural areas did not reveal diffi culties with cognitive and psychosocial functioning. In some patients, however, we observed more diffi culties in verbal tasks and existing major learning diffi culties. Cancer survivors living in rural areas in our region may be more unprivileged due to poor additional supporting services.

Opis:
Introduction. Late cardiovascular complications are the leading causes of morbidity and mortality in patients treated for common malignancies of childhood. Late cardiotoxicity include increased development of atherosclerosis and atherosclerosis – related diseases. An evaluation of the endothelium can be made based on the measurement of endothelium-derived blood vasoactive factors, such as cytokines and adhesion molecules. Their elevated serum levels may serve as sensitive indicators of early atherosclerotic lesions in high risk patients. Currently, assessment of common carotid intima-media thickness has emerged as one of the more powerful tools for evaluation of subclinical atherosclerosis. The purpose of this study was to compare these parameters between patients after antineoplatic treatment compared to persons not exposed to such factors. Methods. Early progression of atherosclerotic disease was evaluated in 64 survivors treated for Acute Lymphoblastic Leukaemia (ALL) in childhood, and in a control group of 36 healthy volunteers. Blood serum concentrations of selected new biomarkers, indicative of endothelial damage and inflammatory activity, were measured, including intercellular adhesion molecule-1 (sICAM-1), endothelial leukocyte adhesion molecule-1 (E-selectin), thrombomodulin (TM), interleukin 6 (IL-6), and high-sensitivity C-reactive protein (hs-CRP). The common carotid intima-media thickness (IMT) was also assessed via ultrasound examination. Results. Significantly higher blood concentrations of sICAM-1 adhesive molecule (229.3±62.2 ng/mL vs. 199.9 ± 63.3 ng/ mL, p=0.0072) and IL-6 (2.1 ± 2.7 pg/mL vs. 1.9 ± 3.6 pg/mL, p=0.0414) were found in ALL survivors compared with control subjects. Concentration of hs-CRP was also higher in the ALL group: 1.3 ± 2.2 ug/mL vs. 0.6 ± 0.9 ug/mL. This difference was close to statistical significance (p=0.0599). The mean IMT values for right and left carotid arteries were higher in ALL patients after antineoplastic therapy, compared with healthy subjects (IMT-R 0.056±0.008 mm vs. 0.052±0.003 mm; p=0.0021; IMT-L 0.057±0.009 mm vs. 0.052±0.003 mm; p=0.0051). Conclusion. Survivors of childhood ALL in the examined group demonstrated elevated concentrations of selected new biomarkers and increased IMT values, compared to controls, which may confirm the occurrence of endothelial injuries in blood vessels. This study indicates that subjects treated for childhood malignancy are at a higher risk of prematurely developing atherosclerosis.

Introduction and objective. The aim of the article is to present the consequences of traumatic brain injury in children, associated with general cognition and behavioural disorders, mainly of the anti-social type. Materials and method. A total of 20 school-age children took part in the study – 6 girls and 14 boys; average age of the children – 13.35 years (standard deviation SD = 1.95). The research instruments included an analysis of documentation, a structured clinical interview, MMSE and Frontal Behavioral Inventory (FBInv) with an additional set of 5 supplementary questions directed at the detection of anti-social behaviour. Results. The functioning of the children with traumatic brain injury is severely disrupted because of the presence of cognitive impairment; however, dementia was not manifested. In a significant number of the children with traumatic brain injury, not only frontal syndrome was found, but also the occurrence of anti-social behaviour. The most commonly reported behavioural problems were: disorganization, commonly referred to as laziness, hypersensitivity, and anxiety. The most common types of anti-social behaviours were: impulsivity, physical and verbal aggression, as well as outbursts of anger. Conclusions. The children with traumatic brain injury suffer from cognitive disorders and behavioural problems, especially impulsivity, physical and verbal aggression, increased anxiety, and disorganization. The occurrence of frontal syndrome is related to the development of anti-social behaviour.

Introduction. Acromegaly is an endocrine disorder caused predominantly by pituitary adenoma leading to autonomic oversecretion of growth hormone and secondary elevation of insulin-like growth factor 1 (IGF-1). Consequently, there are both theoretical and experimental grounds for establishing a correlation between this disorder and the higher incidence of neoplasms. Objective. The aim of the study is to evaluate the incidence and types of neoplasms among patients with acromegaly. Materials and method. The study included 67 patients with acromegaly, aged between 24 and 75±18.8 years, 46 women (68.7%) and 21 men (31.3%), BMI: 30.7±5.7 kg/m2, age at diagnosis 49.1±12.5 years, with the medians of GH and IGF-1 levels at diagnosis of 11.3 ng/ml and 663.8 ng/ml, respectively. A retrospective analysis of medical records with particular regard to physical examination, medical history, laboratory and imaging tests was performed. Results. Fifty-one patients (76.1%) suffered from at least one neoplasm, among whom 48 patients (71.6%) had benign proliferations, whereas malignant neoplasms (larynx, endometrial and colon cancers) were found in only three patients (4.5%). Conclusions. Benign neoplasms were found in majority of patients with acromegaly (71.6%) most notably: nodular goiter and colon polyps; malignant lesions were rare (4.5%). Only every fifth patient suffered from no neoplastic proliferations. No correlations between the studied parameters and the incidence of neoplasms were found, most likely due to the small number of patients. This is the reason for proposing the creating of the first national register of incidences of neoplasms among acromegalic patients.
