Introduction. On 11 March 2020, the Director-General of the World Health Organization (WHO) announced COVID-19 (Coronavirus Disease 2019) as a global pandemic Currently, no vaccines are available and there is little evidence of the efficacy of potential therapeutic agents. Furthermore, there is presumably no pre-existing immunity in the population to the new coronavirus, and it is as-sumed that everyone in the population is susceptible. Objective. The aim of the procedures described in the article is to minimize the risk of human-to-human transmission of the SARS-CoV-2 (Severe acute respiratory syndrome – coronavirus 2) virus during procedures carried out in endoscopic laboratories. Brief description of the state of the art. SARS-CoV-2 infection can be asymptomatic, cause severe pneumonia, or lead to death. Symptoms of COVID-19 range from none (asymptomatic) to severe pneumonia and it can be fatal. Case studies to-date indicate that this infection causes a mild illness (i.e. pneumonia or mild pneumonia) in approximately 80% of cases, and most cases recove; 14% have a more severe illness, 6% experience a critical illness. The vast majority of the most serious illnesses and deaths have occurred among the elderly and people with other chronic underlying diseases. Average progression times include: • in mild cases: from the onset of symptoms to recovery in almost 2 weeks; • in severe cases: from the onset of symptoms to recovery in 3–6 weeks, and from symptoms to death in 2–8 weeks. Conclusions. Special precautions should be taken and procedures followed when performing invasive medical procedures in endoscopic laboratories in patients with specific or clinically probable SARS-CoV-2 infection. Introduction. Prion diseases are slow-acting, neurodegenerative diseases found in humans and many species of animals. Although they occur very rarely in humans, currently, an increase in this type of disease is being observed, probably as a result of exposure to infectious prions causing BSE disease in cows. Objective. The aim of the procedures described in the article is to minimize the risk of human-to-human transfer of all forms of transmissible spongiform encephalopathy, including variant CJD (vCJD) by contaminated medical equipment. Brief description of the state of knowledge. All diseases caused by prions, referred to as transmissible spongiform encephalopathies, are fatal. They are characterized by a long development period (up to several decades). Enormous problems are connected with the risk of transferring prions from patient to patient on the surface of instruments used in medical procedures. Laboratory tests indicate that standard disinfection and sterilization procedures may be insufficient to completely remove infectious proteins from contaminated instruments. One of the methods of infection prevention involves taking equipment used for surgery within the brain, tonsils or appendix, into quarantine until biopsy results of these organs have been received that exclude, as far as possible, asymptomatic carriage of prions. Conclusions. Whenever possible and justified, disposable-use instruments should be used for invasive surgery in patients with definite, clinically probable cases of CJD (vCJD). After use, these instruments should be incinerated.

High-dose chemotherapy currently used in the treatment of children with cancer may induce late effects on psychosocial and cognitive functioning in some of them. The aim of the study was to evaluate psychosocial functioning of childhood cancer survivors living in rural regions. In total, 29 children entered the study. The children were diagnosed as having cancer between 1993-1995 in a single centre. Each patient was examined 5 years after the completion of cancer therapy. The children had been diagnosed with leukemia and lymphoma (72.4%), and solid tumours. Self-report questionnaires, as well as standardized psychological test (WISC-R, WAIS-R-PL) were performed for the evaluation of psychosocial and cognitive functioning of the patients. Most of the studied cancer survivors living in rural areas did not reveal diffi culties with cognitive and psychosocial functioning. In some patients, however, we observed more diffi culties in verbal tasks and existing major learning diffi culties. Cancer survivors living in rural areas in our region may be more unprivileged due to poor additional supporting services.

Introduction. Every farming method, whether conventional or organic, has been associated with some sort of risky behaviors leading to health issues among farmers. Substantial evidence is not available in the literature to determine whether the magnitudes of health outcomes vary between conventional and organic farmers. The study investigated whether selfreported neurological and mental health symptoms differ between conventional and organic farmers living in Indiana, USA. Materials and method. A self-reported questionnaire survey collected information from 200 conventional and 157 organic farmers of Indiana on demographic characteristics, depression and neurological symptoms. Statistical analyses were conducted to observe the differences in self-reported symptoms by groups of farmers. Results. It was observed that the conventional farmers had significantly higher age-adjusted mean neurological symptom score (p<0.01) than the organic farmers. Regression models revealed positive and significant associations of conventional farming with total (β =1.34; p=0.02), sensory (β =0.83; p=0.001) and behavioural (β =0.09; p=0.03) symptoms after accounting for age, income, education and years in farming. Positive but non-significant associations were also observed in conventional farmers with cognitive and motor symptoms, and with all subscales of depression symptoms in the adjusted models. Conclusion. The findings obtained suggest the importance of a larger study to further explain the difference in mental and neurological health effects in these two categories of farmers.

Introduction: According to the general idea a doctor can start the medical management process in an adult and not legally incapacitated patient after the patient has given consent to initiate such a process. The patient’s refusal makes rendering medical services impossible, irrespective of their scope and kind. It should be emphasized that such a refusal is respected if it is expressed fully, clearly and consciously. Cases in which such a refusal is expressed by an intoxicated suicidal patient, remaining under the influence of narcotics, drugs or medicaments which characterize with a similar activity should be particularly analysed. Although such a person is able to verbally declare his objection, his ability to process the information given by the doctor before initiating medical procedures is limited, or even non-existant. The refusal therefore cannot be regarded as reliable, which results in rendering medical services to the patient. Materials and methods: An analysis was made of Acts of Law and the opinions of the judiciar by comparing and excluding contradictory and incoherent elements. Results: Despite the lack of clear regulations of a patient rejecting procedures aimed at saving the patient’s life, or the prevention of serious health impairment or sustaining injury, it should be assumed that the objection expressed by the patient who does not demonstrate the ability to process the information provided by the doctor is not reliable, and the doctor is therefore still obliged to render medical services. External factors, such as consumption of alcohol, narcotics and drugs, which characterize with a similar activity impair perception and make the taking of a conscious decision impossible. Not providing medical help and introducing direct compulsion would mean neglecting provision of due diligence in the process of treatment and, as a consequence, placing the patient’s health at risk, and suffering from negative implications for the patient’s life and/or health in the future. Conclusions: Current provisions should directly regulate the negligence of respecting a refusal expressed ‘unconsciously’ by a patient who remains under the influence of alcohol, narcotics, drugs or medicaments which characterize with a similar activity. Moreover, apart from legal provisions, the law providers should consider introducing a direct compulsion in patients who are unable to make a conscious decision in the treatment process.

Introduction. The operating theatre is one of the most important places in a hospital. Due to the presence of numerous reservoirs of microorganisms and the invasiveness of surgical procedures it is necessary to ensure high hygiene standards in these locations. Objective. The aim of the study was to carry out a qualitative assessment of the microbiological cleanliness of the surfaces and equipment in an operating theatre. Materials and method. The results of microbiological tests of the surfaces and equipment of the Children’s Operating Theatre in Clinical Provincial Hospital No. 2 in Rzeszów, southeast Poland, during 2007–2012 were reviewed retrospectively. Results and conclusions. For the analysis, a total of 1,819 swabs were collected, of which 1.05% were positive. Positive results were obtained mainly from samples taken from moist places (57.9%). Among the microorganisms isolated, Gram- negative bacteria constituted the majority (57.9%), Pseudomonas bacteria were found most frequently (31,6%). Isolated microbes can be the etiological agent of nosocomial infections.

The objective of the research was to determine the effect of habitat type of selected species of trees on the nature of cooccurrence of Fe, Mn, Zn, Cu, Pb, Cd, Ni, Cr and Co. The presence of speciation forms of these metals was investigated, with reference to the species composition of tree stands in selected areas of the Tatra Mountain National Park (Chochołowska Valley, Strążyska Valley, Kościeliska Valley, as well as Mała Łąka Valley). Contents of selected metals in samples were determined by the flame ASA method, with an accuracy of 0.1 μg/g. In habitats dominated by maples, the Pb content in the Chochołowska Valley, unlike Kościeliska Valley covered with beeches, the Pb content in the form directly bioavailable, was twice as high. This was clearly proved in the case of Strążyska Valley where the soil in beech tree habitats contained larger quantities of exchangeable forms of Pb, than that in the Chochołowska Valley. The soil of the valleys, including the Mała Łąka Valley, showed peculiar characteristic averaging of the contents of selected speciation forms of metals in the soil. Content corresponding to 10 percentile and geometrical average may be regarded as benchmarks in future studies of the Tatra Mountain National Park, or other protected areas.

Although smallpox was eradicated over 30 years ago, the disease remains a major threat. High mortality, high infectivity and low resistance of the contemporary population make the smallpox virus very attractive to terrorists. The possible presence of illegal stocks of the virus or risk of deliberate genetic modifications cause serious concerns among experts. Hence, it is reasonable to seek effective drugs that could be used in case of smallpox outbreak. This paper reviews studies on compounds with proven in vitro or in vivo antipoxviruses potential, which show various mechanisms of action. Nucleoside analogues, such as cidofovir, can inhibit virus replication. Cidofovir derivatives are developed to improve the bioavailability of the drug. Among the nucleoside analogues under current investigation are: ANO (adenozine N1-oxide) and its derivatives, N-methanocarbothymidine [(N)-MCT], or derivatitives of aciklovir, peninclovir and brivudin. Recently, ST-246 – which effectively inhibits infection by limiting release of progeny virions – has become an object of attention. It has been also been demonstrated that compounds such as: nigericin, aptamers and peptides may have antiviral potential. An interesting strategy to fight infections was presented in experiments aimed at defining the role of individual genes (E3L, K3L or C6L) in the pathogenesis, and looking for their potential blockers. Additionally, among substances considered to be effective in the treatment of smallpox cases, there are factors that can block viral inhibitors of the human complement system, epidermal growth factor inhibitors or immunomodulators. Further studies on compounds with activity against poxviruses are necessary in order to broaden the pool of available means that could be used in the case of a new outbreak of smallpox.

Introduction. Overweight and obesity, as well as a gonadal function, are pivotal factors influencing bone tissue metabolism. Materials and method. The aim of the study was to determine the effect of dietary induced obesity (DIO) on bone tissue metabolism in sham-operated (SHO) or ovariectomized (OVX) adult female Wistar rats. Additionally, the influence of DIO in SHO or OVX on the concentration of sclerostin in the blood serum was analyzed. After SHO or OVX, the rats were placed in groups (n=8) and either received a standard diet (11.5 MJ/kg) (SHO-CON; OVX-CON) or a high-energy diet (17.6 MJ/kg) (SHO-FAT; OVX-FAT). The experiment lasted for 90 days and allowed for the establishment of osteopenia in OVX females and obesity in the rats that had received the high-energy diet. Results. The results of the study demonstrate that obesity or/and ovariectomy increases the resorption of femora and tibiae, hence decreasing the densitometric and mechanical parameters affecting the bone structure in adult females rats. The strongest osteodegenerative effect was seen in the OVX-FAT females. Interestingly, the degree of bone tissue degradation caused exclusively by ovariectomy was similar to that found in the obese sham-operated rats. Conclusions. Bone losses invoked by DIO seem to be independent from the Wnt/β-catenin pathway inhibition induced by sclerostin. While further study is necessary, the obtained results suggest that the usage of sclerostin anti-body in the treatment of osteoporosis can be ineffective, and in obese patients the undertaking of such therapy should be reassessed.

Many environmental factors, including pesticides, cause immunological system disorders by inducing changes in humoral and cellular response. They may stimulate or trigger immunological autoaggression, hypersensitivity and allergy, as well as lead to immunosuppression, thus increasing the incidence of infectious diseases and cancers. Such activity is also attributed to organophosphorus compounds used in agriculture as insecticides, and commonly in households as biocides. The aim of the study was to define possible mechanisms of the immunotoxic activity of the chlorpyrifos (an organophosphorus compound) on experimental animals following their exposure to the compound via the oral route. The present study attempts to define the influence of chlorpyrifos on the profile of subpopulations of immunoactive cells: B, T, CD4+, CD8+, and NK, and on their phagocytic activity in an experimental in vivo model. For this purpose, the Wistar rats, were exposed orally to increasing doses of chlorpyrifos: 0.1 LD50, 0.15 LD50, 0.2 LD50, 0.3 LD50 and 0.4 LD50 for 28 days. In the study animals, we failed to demonstrate a statistically significant decrease in the phagocytic activity of the granulocyte.
