Opis:
Objective: Farming is associated with exposure to a wide variety of risk factors including organic dusts, endotoxins, allergens and other chemicals. The ability of some of these agents to interact with the immune system is demonstrated in the presented study which was undertaken to evaluate the relationship between pig and cow breeding, and the immune system early changes. Particular attention is paid to selected serum cytokines. Methods: Sixty four animal breeders (36 cattle and 28 pig breeders) were selected as the exposed group, and 32 rural workers not engaged in animal breeding were utilised as the controls. Personal data were collected through a questionnaire, and selected serum parameters measured, including cytokines IL-6, IL-8, IL-10, IFNγ and TNFα, immunoglobulins and proteins, and total and differential white blood cell counts. Results: The study stresses the significant increase of TNF-α, IL-8, and IL-10 in animal breeders, with the highest values in pig breeders, and a slight but statistically significant increase in albumin and total serum proteins. Conclusions: The findings of the presented study suggest a condition of immune system activation in animal breeders, with the highest levels observed in pig breeders. These changes may be attributable to exposure to organic dusts, endotoxins, or to the different biological agents present in the rural environment. The prognostic significance of these findings, however, remains unclear, but the observed changes might be indicative of a risk of developing respiratory toxic and allergic diseases, which need to be further investigated.

Opis:
The aim of current studies was to validate the direct plating of a stool sample for Listeria monocytogenes detection, using selective medium Palcam agar with Palcam selective supplement. Validation was performed using stool samples collected from healthy humans inoculated with Listeria sp. strains. Stool samples were frozen to determine the influence of freezing on method robustness. The presented research defines the Listeria monocytogenes limit of detection (LOD) as 103 cfu/g of stools for fresh and frozen samples. Repeatability and reproducibility of the method has been confirmed using statistical methods. We show the effectiveness of direct plating of stool samples on Palcam agar with Palcam selective supplement collected for Listeria monocytogenes detection. This method could be useful for this pathogen detection in stool samples collected from patients with diarrhoea.

Despite urbanization of contemporary society and limitation on the environmental areas, the threat of animal attacks on people is still an important social and medical problem. Although they usually do not have serious consequences, they are nevertheless disturbing and alarming enough to make people seek professional, medical help. The aim of this study was to estimate the epidemiology and the risk rate of animal related injuries in the urban and rural population, as well as to select the characteristics of such injuries, which are distinguishable from injuries caused by other factors. The work presents a retrospective analysis of the medical records of 1,872 patients treated at the Department of Trauma and Emergency Department between 2001-2004, who suffered from animal related injuries. Our analysis led to the following conclusions: Most animal related injuries, reported to doctors, are mild injuries. The threat of animal attack is similar in urban and in rural areas, and concerns mostly men between the ages of 22-40. Injuries caused by large animals, such as a horse or a cow, require hospitalization and should be considered as high-energy injuries. The vast majority were patients who after the treatment could return to their social and occupational activities; the mortality rate in hospitalized patients with animal related injuries was higher then in other injuries, but the difference was statistically irrelevant.

Opis:
Introduction and objective. Working at night and in shifts, as well as an unhealthy lifestyle, may increase the risk of breast cancer in nurses who therefore should frequently perform breast self-examination (BSE). The aim of the study was to investigate the performance of BSE among Polish nurses, its accuracy, sources of knowledge and skills, self-assessment of own competences and preparation to educate women about BSE. Materials and method. A descriptive, cross-sectional study was conducted in a group of female nurses (N=1,242). An anonymous, self-administrated questionnaire was used for data collection. To properly assess the BSE a four-item scale was made of the type single best answer multiple choice question. Each item referred to one agreed principle of performing BSE. Results. Regularly BSE was performed by 56.1% nurses, 67.3% pre-menopause nurses 2–3 days after cessation of menstruation and 30.4% post-menopause women on a chosen day of the month. About 98% examined visually and by palpation; 58.9% did so in two positions. In the accuracy scale of BSE, the average number of points was 2.8 out of 4. All (4 points) or almost all (3 points) recommendations of accurate BSE were met by 61.4% of the nurses. Self-assessment of knowledge and BSE practical skills were considered as good or very good by 93.5% and 88.8% nurses, respectively. The self-evaluation of nurses’ knowledge and BSE skills was significantly correlated with the result on the accuracy scale or this BSE. Conclusions. Many deficiencies concerning frequency, times and BSE techniques were revealed among Polish nurses. There is a discrepancy between the high self-assessment of competences and the accurate practice of BSE. Nurses’ preparation in Poland in BSE is insufficient and requires improvement.

Opis:
Introduction. Prostate cancer (PC) is the most commonly diagnosed malignant tumour and the third cause of cancer deaths among men in Europe. The treatment of early-stage PC is very effective and in many cases allows achievement of a complete cure, whereas the treatment of metastatic prostate cancer (mPC) is still a huge challenge for clinicians. New therapeutic strategies for mPC are urgently needded. One of the most promising methods of treatment is anticancer immunotherapy including the monoclonal antibodies against immune checkpoint inhibitors. Objectives. To present the potential possibilities of using checkpoint inhibitors blockage in the treatment of mPC, and to overview the results of recent research on immune checkpoint inhibitors in patients with PC. State of knowledge. Recent studies suggest that monoclonal antibodies directed against immune checkpoint inhibitors in combination with traditional therapy may become a breakthrough in the treatment of mPC in the near future. Conclusions. The immunotherapy using monoclonal antibodies against immune checkpoint inhibitors seems to be a new opportunity for patients with advanced PC. The key to achieve the maximum anti-tumour response is to choose the best candidates for this therapy and determine the optimal sequence and combination of drugs. The introduction of immunotherapy as the standard treatment of patients with advanced PC requires further studies.

Opis:
Introduction and objective. The male reproductive system constitutes a set of tissues which are particularly sensitive to external factors. The aim of the study was to analyze the relationship between background radioactivity and the quality of the semen of the men who have reported to the infertility treatment facility in 2000–2016 in the Lublin region of eastern Poland. Materials and method. The radioactivity of the ground-level air layer obtained from the Institute of Meteorology and Water Management in Włodawa in the Lublin region was analysed. 4,250 spermiograms of patients who reported to the treatment facility for the first time due to infertility were analysed and correlated to background radioactivity in the Lublin region in 2000–2016. Results. A long-term decrease in the percentage of morphologically normal spermatozoa of the patients was observed (r=-0.970; p<0.001 in 2000–2009 and r=-0.925; p=0.003 in 2010–2016). Men’s age correlated negatively with sperm motility (r=-0.164; p=0.009) and morphology (r=-0.186; p=0.009). The percentage of spermatozoa with normal morphology was lowered by beta-isotopes of artificial origin in the air in 2000–2009 (r=-0.655; p=0.040) and by the exposure to gamma radiation of the ground-level atmosphere in 2010–2016 (r=-0.676; p=0.048). The percentage of sperm vitality was lowered by gamma radiation in the atmosphere (r=-0.636; p=0.006), but improved by beta isotopes in precipitation (r=0.686; p=0.002) in the whole of the analyzed period. Conclusions. The percentage of morphologically normal spermatozoa in patients who reported to the infertility treatment depends not only on the age of patient, but also on beta-isotopes of artificial origin and gamma radiation in the atmosphere. Beta isotopes in precipitation affect the improvement of sperm vitality.
