Opis:
Introduction and objective. Stress at the work place reduces efficiency, as well as increasing accidents and absences, which may cause various diseases, including cardiovascular diseases. The aim of the study is an analysis of the prevalence and causes of stress in intellectual work, and its correlation with the prevalence of cardiovascular diseases in women at non-mobility working age. Materials and method. The study was conducted in 2016–2017 in a group of 300 women aged 45–60 in non-manual employment. A questionnaire for subjective job evaluation was used. Logistic regression models for the occurrence of CVDs versus frequency of occurrence of individual causes of work stress among the respondents were estimated. Results. Women at non-mobility working age with non-manual employment are especially exposed to stress, half of them experience high level of stress at work, most often caused by social contacts and lack of rewards and support. Cardiovascular diseases were found in 26.5% of the women studied who were also significantly higher exposed to the occurrence of health hazards due to exposure to harmful factors, or due to an accident at work, changes in the workplace, the need to compete with others, and the need to perform the task despite the lack of appropriate material resources, compared with women without such diseases. Conclusions. The study revealed a high prevalence of occupational stress in non-manual employment, indicated its main causes that correlated with the alence of cardiovascular diseases. Actions are needed to reduce the level of stress in the work of women at non-mobility working age, to maintain their work ability and quality of life.

Opis:
Introduction. Iodine deficiency is one of the most important public health problems in the world. It mostly affects pregnant women and children. The lack of iodine leads to an increase in the numbers of perinatal mortality and mental retardation. Materials and method. In 2012–2014, a survey was carried out of 2,342 women, of whom 2284 were analyzed for salt to measure the content of iodine, and 2,242 women of reproductive age had their urine samples analyzed by the ceriumarsenite method based on the Sandell-Kolthoff reaction. Determination of the medical and social effectiveness of biological monitoring of IDS was carried out in 3 stages. Results. The study revealed that in the eastern Kazakhstan region the relative risk (RR) of overall prevalence of iodine deficiency among women was 1.1 times higher (95% ID: 0.35–3.61; Z statistic = 0.192) among household respondents (≤15mkg/kg). In the Pavlodar region, the RR of the prevalence of total iodine deficiency is 0.5 times higher in households where the salt was not sufficiently iodized. In the Zhambyl region the RR of the iodine deficiency prevalence was about twice as high due to insufficient iodized salt. Conclusions. The introduction of biological monitoring of IDS among women, especially pregnant women, may positively affect the detection of the risk limits for the birth of infants with reduced mental abilities or other diseases during pregnancy, using timely targeted preventive measures to exclude factors that affect the spread of IDS.

Opis:
Introduction. The ageing society of Poland still experiences health-related problems and increasing difficulties in receiving medical care. The initiation of telemedicine programmes can change this situation for the better. The question arises whether telemedicine is a solution that the elderly are willing to accept and take advantage of in order to raise their quality of life. Objective. The aim of the research was an investigation of the quality of life and the course of ageing among the elderly Polish population, and their openness to the notion of telemedical services. Materials and method. A questionnaire-based study was conducted in 2015 on a sample group of 312 seniors. The respondents completed the Diener Satisfaction with Life Scale (SWLS) and the subscale of Health States from Fragebogen zur Lebenszufriedenheit (FLZ) by Fahrenberg et al. Each participant gave consent to take part in the research. Results. SWLS presents a relatively stable value in the subsequent decades of old age (60–69 years, M=21.93, SD=6.25; 70–79 years, M=21.70, SD=5.52; 80+yeasrs, M=21.38, SD=5.82). The health related quality of life (the FLZ subscale) varied in the analysed subgroups (F=7.783: p=0.000) and was related to comorbidity and polytherapy. A positive attitude towards telemedicine was expressed by 40% of those surveyed. However, the need of the seniors for telemedicalservices was limited and did not exceed the 10%. Conclusions. In Poland, in ageing there still dominates the experience of multiple ailments and polytherapy, something which justifies a search for new, easily accessible, and economically sound solutions for health care. Telemedicine is a promising solution, although there is a need for definite steps to be taken to increase the willingness among geriatric patients to take advantage of telemedicine.

Opis:
Aim. The purpose of this study was to analyze the hospitalization of diabetes mellitus patients in Polish hospitals in 2005-2009. Materials and Methods. Data was taken from a nationwide database, kept at the National Institute of Public Health – National Institute of Hygiene in Warsaw. Data gathered for this work related to patients whose main cause of hospitalization was diabetes. Results. In the period of five years the number of diabetes-caused hospitalizations increased by nearly 22% – from 172.2 per 100 thousand in 2005 to 209.9 per 100 thousand in 2009. Hospital treatment covered mainly patients suffering from type 2 diabetes (116.4 per 100 thousand in 2009), as well as type 1 diabetes (87.6 per 100 thousand in 2009). Patients under 39 years of age were more often hospitalized because of type 1 diabetes, whereas in the older age groups patients were more often treated in hospitals for type 2 diabetes. Generally, in both types of the disease, older patients required hospitalizations more often than the younger ones. Cardiovascular diseases were the most reported co-morbidity in both types of the disease. In 2005-2009 the hospital mortality rate decreased with regard to both types of diabetes and an average length of hospital stay decreased by one day, reaching 8.1 days in 2009. Conclusion. It must be emphasized that the growing epidemic of diabetes and its complications are an important challenge to society. The percentage of people hospitalized due to diabetes is increasing every year and consumes significant resources dedicated to health care. Early diagnosis and appropriate treatment of diabetes are imperative, as well as reducing the disparities in access to medical care (ambulatory and stationary) for town and country residences.

Opis:
Toxocariasis is a helminthozoonosis due to the infestation of humans by roundworms, Toxocara spp. Actual informations indicate it the most common worm infection in many countries, typically connected with rural areas. The authors analyzed the documentation of 84 children with positive serology to this worm. An individual record was made and following data were restricted: anamnesis data, clinical symptoms, epidemiological data, eosinophils number, level of immunoglobulins G and E. The highest Toxocara spp. seropositivity frequency was found in the schoolchildren aged 7-10. The most frequent clinical findings in children infected Toxocara spp. were lymphadenopathy, hepatomegaly, arthralgia and arthritis. 15.5% of seropositive patients presented non-specific symptoms originating from the central nervous system: headaches, sleep and behavioural disorders, and hyperactivity. The mean eosinophilia in the peripheral blood was detected in the youngest children: 4,023 cell/μl, which is 15.55 times more than the limit value. Hyperimmunoglobulinemia E was detected in all age groups, and the youngest children presented a serum concentration of IgE that was 16.47 times higher than the limit value. Conclusions: 1. Toxocara spp. infection is detected in children at every age, but the most specific age group are schoolchildren, representing 38% of positive individuals. 2. The clinical spectrum of toxocariasis reflects various manifestations depending on the internal organs infected by the migrating worms and the intensity of infection. 3. Eosinophilia seems to be a good marker of infection in young children who have a more symptomatic course of the disease. 4. Hyperimmunoglobulinemia IgE can be the important element which distinguishes between current and past Toxocara infection, but its meaning is not connected with the age of infected children

somatotropic pituitary hypofunction

Opis:
Introduction. Growth hormone deficiency (GHD) is one of the main indications for growth hormone therapy. One characteristic of this disease is bone age delay in relation to the chronological age. Pituitary dysfunction negatively affects the growth and development of the jaws and teeth of the child. The secretion of endocrine glands regulates growth, development, and gender differentiation. It also controls the growth of bones and teeth, regulates metabolism of calcium and phosphate, proteins, lipids and carbohydrates. The primary role in the endocrine system is played by the pituitary gland which is responsible for the production of somatotropin[1]. Dysfunction of the pituitary gland has a negative effect on the growth and development of long bones in the body, and may have an adverse effect on the development of maxilla, mandible and dentition of a child. There is some information in the literature that dental age is delayed in short stature children; the replacement of deciduous teeth by permanent teeth is also delayed, and newly erupted permanent teeth often require orthodontic treatment. Applying hormonal therapy positively affects the process of replacement of dentition [2, 3, 4, 5, 6]. Objectives. The aim of the study was to assess bone and dental age, as well as analyze the state of dentition in children diagnosed with GH deficiency treated with growth hormone, depending on the duration of treatment. Materials and method. The study material consisted of 110 children (27 males, 83 females), hospitalized for somatotropin hypopituitarism in the Department of Paediatric Endocrinology and Diabetology at the Medical University of Lublin, Poland. The mean birth age was 13 years (156 months) with a standard deviation of 2 years and 6 months (30 months). 47 children (43%) started treatment with the growth hormone (group starting treatment) and 63 children (57%) whose treatment was started 2–3 years previously (group in the course of treatment). The control group consisted of 41 generally healthy children (15males, 25 females) with ENT problems, such as hypoacusis and a condition after nasal injury, hospitalized in the Department of Paediatric Otolaryngology at the Medical University of Lublin, Poland. The mean age was 11 years and 5 months (137 months) with standard deviation of 2 years and 5 months (29 months). Informed consent was obtained from the parents. The study was approved by the Bioethical Committee at the Medical University of Lublin (Resolution No. KE-0254 /216 /2012).
