The aim of the study was to investigate the occurrence of serum antibodies to C. psittaci in workers at 8 stock farms located in a rural area of eastern Sicily. Serum samples from 188 workers and 160 controls were tested for immunoglobulin IgA, IgG and IgM direct against C. psittaci by microimmunofl uorescent test (MIF). Seropositive subjects were defi ned as those with IgG titres of 1:16-1:256, and seronegative subjects as those whose titre was <1:16. To rule out the presence of cross reactive antibodies, testing was also performed for C. trachomatis and C. pneumoniae. A total of 28 (14.9%) subjects studied met the criteria for seropositivity to C. psittaci. Two of the 188 (1.06%) had an IgG titre of 1:32, 16 (8.51%) an IgG titre of 1: 64, 6 (3.19%) an IgG titre of 1:128 and 4 subjects (2.12 %) demonstrated an antibody titre of 1:256. The high prevalence rate of C. psittaci antibodies among farmers suggests that this infection is spread in those subjects living in areas with an high percentage employed in rearing activities. The authors stress the importance of carrying out health surveillance in subjects working in close contact with animals receptive to infection, and confirm the need to adopt a serological test, such as MIF as a preventive measure for activities at risk.

Introduction. In Poland, an increasing number of patients are hospitalized due to liver diseases. One of the common liver diseases is cirrhosis, which can be caused by alcohol, viral hepatitis, autoimmune processes and metabolic diseases. Materials and method. The study included 99 patients with alcoholic cirrhosis from the Lublin region of Eastern Poland. The control group consisted of 20 healthy individuals without liver disease who did not abuse alcohol. The concentrations of serum kallistatin and chemerin were determined using ELISA kits. Objective. The aim of the study is to evaluate serum levels of kallistatin and chemerin in patients with different stages of alcoholic liver cirrhosis. Results. The highest chemerin level was found in the control group – 182.6±80.4 ng/ml. In other stages of liver cirrhosis, the following levels were observed: 175.7±62.7 ng/ml in Child-Pugh stage A (Ch-P A), 150.2±59.7 ng/ml in Ch-P B and 110.3±73.6 ng/ml in Ch-P C. Significant differences in chemerin levels between controls and Ch-P C patients (p=0.01), as well as between the Ch-P A patients and Ch-P C patients (p=0.02), were demonstrated. The highest kallistatin level was demonstrated in the control group – 8.2±3.5 μg/ml. In other stages of liver cirrhosis, the following concentrations were found: 7.2±27 μg/ml in Ch-P A, 4.4±2.2 μg/ml in Ch-P B and 3.5±1.9 μg/ml in Ch-P C (Tab. 3). Statistically significant differences were observed between controls and Ch-P B patients (p<0.001), controls and Ch-P C patients (p<0.001), Ch-P A and Ch-P B patients (p=0.01), as well as Ch-P A and Ch-P C patients (<0.001). Conclusions. The levels of chemerin and kallistatin decrease with progression of liver damage during alcoholic liver cirrhosis. The impairment of its synthetic function leads to reductions in levels of the adipokines studied.

Introduction and objectives. For years, the increase in cancer incidence and deaths has constituted a significant health and social problem. Variation in the burden in cancers in different regions of the world requires constant monitoring of the epidemiological situation in this regard. Assessing survival in cancer patients is a valuable source of information for patients and physicians alike, as well as for politicians who have a direct impact on the shaping of health policy and health systems. The aim of the present study was to assess the changes in the 5-year relative survival of colorectal cancer patients during 1995–2014. Materials and method. The data of 8,970 patients with colorectal cancer in the years 1995–2014, 5,033 males and 3,937 females aged 67.5 ± 11.7 from Świętokrzyskie Cancer Registry were used. Cases were classified according to the topographical codes ICD-O-3: C18.0-C18.9, C19.9, C20.9, C21.0-C21.2, C21.8. The end of follow-up was fixed at 31 December 2014. Four five-year calendar periods were defined. In each calendar period, relative survival rates using the Ederer II method were estimated separately for males and females. Results. In 2010–2014 (against 1995–1999), the absolute increase in the 5-year relative survival in males and females with colon cancer was the highest and reached 9.8 percentage point (p.p.) and 9.6 p.p., respectively. Patterns of survival for both colon and rectal cancer patients according to gender and age were very similar. Conclusions. In 1995–2014, an increase in the value of relative survival rates of males and females with colorectal cancer was observed. Systematic increase in funding in health care was a chance for reducing the burden of colorectal cancer by more widespread and equal access of effective early detection and cancer treatment.

Introduction. The problem concerning the impact of pregnancy on diabetic complications is a matter for discussion as there is some evidence suggesting that pregnancy may trigger development or progression of diabetic chronic complications. However, currently available data concerning this issue is still controversial. Objective. The aim of the study was to evaluate the impact of obstetric history on the development of chronic microangiopatic and macroangiopatic complications in type 1 diabetic women. Materials and Method. The retrospective study comprised 226 white Caucasian type 1 diabetic women, including 190 parous and 36 nulliparous women. Anthropometric data, information concerning the course of the disease, including metabolic control and chronic complications, together with obstetric history, were registered. Results. Parous women were older (p<0.001), but did not differ significantly regarding metabolic control in the course of the disease (p>0.05) and diabetes duration (p>0.05) from nulliparous subjects. There were no significant differences in the incidence (p>0.05) nor onset (p>0.05) of chronic diabetes complications between the groups. The number of deliveries did not correlate with either the incidence nor the onset of chronic complications. Longer DM duration at the moment of first delivery was related to the higher incidence of retinopathy (p<0.01), nephropathy (p<0.05) and neuropathy (p<0.001). Conclusions. The incidence of chronic diabetic complications does not differ between parous women and the subjects that were never pregnant, and is not related to the number of pregnancies.

Objective. The aim of this study is to evaluate neurological scales, as well as biochemical and radiological parameters measured on day 10 after ischemic stroke (IS), according to their value as predictors of the long-term outcome. Materials and method. 45 patients were assessed according to the Barthel Index (BI) and National Institute of Health Stroke Scale (NIHSS) on day 10, and according to Modified Rankin Scale (mRS) 3 months after the onset of IS. On day 10 of IS, the serum level of C-reactive protein (CRP), albumin, D-dimers (DD), S100BB and Tau proteins was measured and the volume of ischemic focus assessed with the use of Computed Tomography (CT). The patients were divided into groups with good outcome (GO) and mRS 0–2, and with bad outcome (BO) and mRS 3–6. Results. NIHSS and BI scores (p<0.001), the volume of ischemic focus (p<0.01), CRP (p<0.01) and albumin level (p<0.05), but not DD, S100BB and Tau protein levels evaluated on day 10, correlated with mRS after 3 months since IS onset. Patients from the BO group were observed to have lower BI (p=0.001), higher NIHSS (p<0.01) and CRP levels (p<0.05), and bigger volume of ischemic focus (p<0.05) measured on day 10 of IS. In the GO group, there were more patients with atherosclerotic etiology (p=0.02 x2=7.856). Regression analysis showed that only the BI score assessed on day 10 of IS can predict the outcome after 3 months assessed by mRS (OR=1.102, 95%, CI:1.01–1.203; p=0.001). Conclusions. BI assessed on day 10 has a predictive value for the outcome evaluated by mRS 3 months after the onset of IS.

Out of the twenty-one A. hydrophila complex isolates obtained during a routine examination of human diarrhoeal faeces, two A. hydrophila subsp. dhakensis isolates (P1097 = CCM 7329 and P1165) were successfully identified by ribotyping. The correct taxonomic position of the A. hydrophila subsp. dhakensis CCM 7329 was verified by cpn60 sequencing (GeneBank accession number HM536193). The remaining A. hydrophila complex isolates were identified as A. hydrophila subsp. hydrophila. The ability of biochemical tests and fatty acid methyl ester analysis to reliably discern both A. hydrophila subsp. dhakensis and A. hydrophila subsp. hydrophila was limited. In contrast to the A. hydrophila subsp. hydrophila, the faecal isolates of A. hydrophila subsp. dhakensis did not produce acid from arbutin. When compared in a two-dimensional plot, the A. hydrophila subsp. dhakensis faecal isolates contained higher amounts of the two minor fatty acids C13:0 and C17:1 ω8c than the A. hydrophila subsp. hydrophila reference strain. This is the first detected occurrence of the less frequent A. hydrophila subsp. dhakensis in our region and ribotyping was proved as a suitable method for the identification of A. hydrophila subsp. dhakensis.

Introduction. Paget’s disease (PDB) is a focal disorder of bone remodeling that occurs commonly in older people with decreasing prevalence reported in European countries. This disease is most often asymptomatic, but it can cause a variety of medical complications resulting in considerable morbidity and reduced quality of life. There is little information regarding the epidemiology of PDB in Poland. To the best of the authors’ knowledge, this is the first large epidemiological analysis of this disease in Poland. Objective. The aim of this study was to analyze factors that may be related to the PDB epidemiology among hospitalized patients in Poland. Materials and method. The analysis was conducted on the basis of population-based administrative data, taken from a Polish hospital morbidity study carried out by the National Institute of Public Health between January 2008 – December 2014. Results. Analyzed data covered 662 hospitalization records. The final study sample comprised 94 (41.8%) male and 131 (58.2%) female patients with first-time hospitalizations for PDB, with a significant predominance of females (P<0.02), and the predominance of patients living in urban (73%) than in rural areas (27%), P<0.001. The average age of the sample was 56.8 years (CI: 54.3–59.3; SD 18.8; range 1–93 years). The number of PDB cases hospitalized in Poland significantly decreased during the analyzed period of time. Conclusions: PDB is a rare disease with decreasing trends observed among hospitalized patients in Poland. The study results may suggest the existence of environmental risk factors for the development of PDB.

Epidemiological studies in Poland show that tobacco smoking by adolescents at reproductive age is still frequently observed. This concerns both boys and girls. The study was based on all-Polish population studies of health behaviours of adolescents aged 14-24 (Youth Behavioural Polish Survey – YBPS) conducted in 2011, and the Pregnancy-related Assessment Monitoring Survey (PrAMS). More than 12% of pregnant women do not discontinue smoking in association with becoming pregnant and expose the foetus to tobacco smoke, despite being aware of the hazardous effect of smoking on the health of the mother and child. Smoking in pregnancy is mainly observed among mothers with a low education level and those aged under 23. According to the Baker’s Foetal Origins of Adult Health and Diseases Hypothesis, exposure of the foetus to the components of tobacco smoke results in many perturbations in the form of a lower birth weight, prematurity, worse state of neonates after birth, and higher susceptibility to contacting civilization diseases at the age of adulthood. The results of studies confirm some observations. Polish studies clearly confirmed a lower birth weight of babies delivered by mothers who smoke; however, earlier termination of pregnancy and worse status of neonates after birth were not observed. According to the Baker’s hypothesis, a lower birth weight of babies delivered by smoking mothers during the further life cycle exposes the offspring to the risk of contracting civilization diseases. The efforts undertaken by public health authorities should be biased towards education of the population at reproductive age about the hazardous effect of smoking on the health of the foetus and the offspring born. Women at reproductive age should be encouraged to discontinue smoking in association with the planning of pregnancy and in pregnancy.

Background: Use of mouthwash and an increased risk of oral cancer has been a source of controversy for decades. A metaanalysis of epidemiological studies of mouthwash and oral cancer and, specifically, mouthwash containing >25% alcohol, was undertaken. Methods: Summary estimates were obtained with maximum likelihood estimates from random effects models. Sensitivity analyses were conducted to evaluate the influence of various inclusion. Results: Eighteen studies were included in the meta-analysis. There was no statistically significant associations found between regular use of mouthwash and risk of oral cancer (RR=1.13; 95% CI (0.95-1.35)). There was no significant trend in risk of oral cancer associated with increased daily usage of mouthwash (p=0.11). There was no association between reported use of mouthwash specifically containing alcohol and risk of oral cancer (RR=1.16; 95% CI (0.44, 3.08)). Conclusions: This quantitative analysis of mouthwash use and oral malignancy revealed no statistically significant associations between mouthwash use and risk of oral cancer, nor any significant trend in risk with increasing daily use; and no association between use of mouthwash containing alcohol and oral cancer risk.

Summaries in 2 parts are presented from a conference held in London at the Royal Society for Public Health on 16-17 May 2012, on the latest developments in dealing with waterborne hospital-acquired infections (nosocomial), from the UK perspective. Also included were some views from continental Europe. The first part, focuses on management strategies and plans that are either in use or recommended by domestic/international guidelines, such as the WHO, for prevention, control and risk assessment of disease outbreaks resulting from the presence of these pathogenic microorganisms in water appliances/supplies. Various solutions are discussed, some more effective than others, but all require a comprehensive strategy and technical expertise run by properly trained and dedicated professionals.
