Purpose: Whiplash injuries to the cervical spine represent a considerable economic burden on society with medical conditions, in some cases persisting for more than a year. Numerous studies of whiplash injuries have been made for occupant normal seated position, leaving the analysis of neck injuries for out-of-normal positions not well documented. For that purpose, a detailed human cervical spine finite element model was developed. Methods:The analysis was made for four most common occupant seated positions, such as: Normal Position with the torso against the seat back and the head looking straight ahead, Torso Lean forward position with the torso away from the seat back for approximately 10°, Head Flexed position with the head flexed forward approximately 20° from the normal position and HeadFlexed with Torso Lean forward position with the head flexed forward approximately 20° and torso 10° from the normal position. Results: The comparative study included the analysis of capsular ligament deformation and the level of S-curvature of the cervical spine. The developed model predicted that Head Flexed seated position and Head-Flexed with Torso Lean forward seated position are most threatening for upper and lower cervical spine capsular ligament respectively. As for the level of S-curvature, the model predicted that Head-Flexed with Torso Lean forward seated position would be most prone to neck injuries associated with it. Conclusions:This study demonstrated that the occupant seated position has a significant influence on potential whiplash injuries.

Opis:
Purpose: The aim of this paper is to present the novel model for the functional spinal unit and spine designed as a rigid mechanism and solve it with methods commonly used in robotics. Method: The structure of the intervertebral joint is analyzed with special attention paid to elements defining the displacements in the joint. The obtained mechanism is then numerically solved using a constraint equations method. Results: The input data set for the simulation is prepared using the 3D scan of the lumbar spine. The simulation results show that the intervertebral joint mechanism can satisfy the ranges of flexion, lateral bending and axial rotation as compared with literature data. It is also possible to study complex, coupled displacements of the lumbar spine segment. Conclusions: Structural analysis of the functional spinal unit with methods common in robotics can eventually lead to better understanding of stabilizing and guiding mechanisms. The proposed mechanism can be used as a reference in the study of spine guidance. It can reproduce the angular displacements of the actual functional spine unit. It is also possible to expand the model to facilitate the analysis of a lumbar spine segment.

Opis:
The purpose of the paper is to present a dynamic model of bicyclist’s lumbar spine for the evaluation of linear and angular variation of intervertebral distance in sagittal plane. Ten degrees of freedom biomechanical model of the spine was solved numerically. Larger loads acting on a cyclist spine occur mostly while sitting in a sport position in comparison with recreation or middle sitting. The load on lumbar spine region is influenced by cycle’s tire pressure, road bumps and wheeling speed. The biggest linear and angular displacements were found between L4–L5 vertebras. The biggest load protractile spine muscle experiences in the sport sitting position. Maximum vertebrae rotation and linear variation values in wheeling regime with 1.5 Bar tyres pressure and at a speed of 10 km/h are 0.46° and 0.46 mm. Maximum vertebrae rotation and linear variation values for a 23 year old, 1.74 m high and 73 kg of mass (bicycle mass ~ 7 kg) man in wheeling regime with 3.5 Bar tyres pressure and at a speed of 30 km/h are 3.9° and 1.23 mm. The biggest variation of rotation in sagittal plane between two nearest lumbar spines is about 1°. Because of this displacement the frontal part of last mentioned disc is compressed with 530 N more and dorsal disc part as much less.

Mechanical properties of cervical spine ligaments are of great importance for an accurate finite element model when analyzing the injury mechanism. However, there is still little experimental data in literature regarding fresh human cervical spine ligaments under physiological conditions. The focus of the presented study is placed on three cervical spine ligaments that stabilize the spine and protect the spinal cord: the anterior longitudinal ligament, the posterior longitudinal ligament and the ligamentum flavum. The ligaments were tested within 24-48 hours after death, under two different loading rates. An increase trend in failure load, failure stress, stiffness and modulus was observed, but proved not to be significant for all ligament types. The loading rate had the highest impact on failure forces for all three ligaments (a 39.1 % average increase was found). The observed increase trend, compared to the existing increase trends reported in literature, indicates the importance of carefully applying the existing experimental data, especially when creating scaling factors. A better understanding of the loading rate effect on ligaments properties would enable better case-specific human modelling.

Previous studies have demonstrated that, the length of the lumbar spine is decreasing with age. Despite considerable research based on sagittal measurements, little is known about the changes in the volume of vertebrae. The objective of this study was to evaluate the changes in the volume of either column of the spine with age. Materials and methods: Computed tomography scans of 62 asymptomatic subjects, performed for thoracolumbar trauma evaluation were used to create virtual 3D models. At least 10 patients were assigned to every decade of life from third to eight. We used a novel technique to measure the volume of anterior column (AC) and posterior column (PC) per each segment (a total of 310 segments). Midline sagittal images were used to measure disc height (DH) and vertebral body height (VH). Results: With age, both DH increases, whereas the VH decreases. The overall length of lumbar segment of the spine decreases with age. The volumetric measurements performed on same subjects showed that volume of both AC and PC does not change with age in females. In males, there is a weak but statistically significant correlation between AC volume and age and no change in the volume of PC. The ratio of PC:AC volume does not change with age in women, although it decreases slightly but significantly (in favor of AC) with age in males. Conclusions: The overall length of lumbar spine decreases with age. This process is not a result of mere changes in the volume of either AC or PC.

For the evaluation of the functionality and mobility of the spine, several methods have been developed. The purpose of this study was to estimate the test-retest reliability of the Spinal Mouse, a new, non-invasive, computer-assisted wireless telemetry device for the assessment of the curvatures, the mobility and the functionality of the spine. Materials and methods: the test-retest reliability was evaluated in 50 adults with back or low back pain. Twenty four parameters were studied in the sagittal and frontal plane. For the characterization of the precision, the intraclass correlation coefficient and the standard error of measurement were used. Results: in the sagittal plane, 22 of the 24 parameters showed high and good reliability, while only two fair and poor. In the frontal plane, 17 parameters showed high and good reliability, five fair and two poor. Discussion: the Spinal Mouse showed excellent test-retest reliability in the sagittal plane, while a slightly inferior performance in the frontal plane, for the evaluation of curvatures, deformation and mobility of the spine.

Opis:
Purpose: This study is to reveal the deformation of intervertebral disc (IVD), the stress distribution of solid phase and liquid phase, the variation of fluid flux and flow velocity in lumbar spine and the influence of different permeability parameters on them under intermittent compressive loading. Methods: A poroelastic FEM of L4-L5 is assigned with different permeability parameters to analyze the deformation, stress distribution and fluid convection under intermittent compressive loads. Results: The results show that the pore pressure of IVD decreases with time, but the effective stress increases under intermittent compressive loads. The axial and radial strain will increase and fluid loss will recover at a more rapid rate if the permeability of endplate increases during unloading period. The velocity vectors show that most of the liquid in the disc flows into vertebrae through endplates and only a small quantity of liquid flows through the annulus fibrosus at the loading step, however, at the unloading step, almost all the liquid flowing into IVD is through the endplates. Conclusions: The changing rate of pore pressure and effective stresses of nucleus pulposus and annulus fibrosus with higher permeability is smaller than that with smaller permeability. The degenerated endplate (with low permeability) yields high flow velocity decreasing gradient, which might impede liquid inflowing/outflowing smoothly through the endplates. The fluid flowing velocity in loading phase is faster than that in unloading phase, so a short resting time can relieve fatigue, but could not recover to the original liquid condition in IVDs.

Opis:
With the aim to study disc degeneration and the risk of injury during occupational activities, a new finite element (FE) model of the L4-L5-S1 segment of the human spine was developed based on the anthropometry of a typical Colombian worker. Beginning with medical images, the programs CATIA and SOLIDWORKS were used to generate and assemble the vertebrae and create the soft structures of the segment. The software ABAQUS was used to run the analyses, which included a detailed model calibration using the experimental step-wise reduction data for the L4-L5 component, while the L5-S1 segment was calibrated in the intact condition. The range of motion curves, the intradiscal pressure and the lateral bulging under pure moments were considered for the calibration. As opposed to other FE models that include the L5-S1 disc, the model developed in this study considered the regional variations and anisotropy of the annulus as well as a realistic description of the nucleus geometry, which allowed an improved representation of experimental data during the validation process. Hence, the model can be used to analyze the stress and strain distributions in the L4-L5 and L5-S1 discs of workers performing activities such as lifting and carrying tasks.
