The aim of our study is to determine the importance of various aspects of delivery and post-natal care for women in the third trimester of pregnancy, and the analysis of relationship of these aspects to selected personal variables also in the context of the relationship with the yet unborn child. In this exploratory study, 94 pregnant respondents participated, including 72% of primiparous and 28% of multiparous women. The participants completed the MFAS questionnaire and a survey in the form of closed questions. The results show that women preparing for childbirth considered as very important medical standards included in the Regulation of the Minister of Health: freedom and the opportunity to decide on issues related to birth and care of a newborn child, as well as care and support from professionals and the loved ones; however, only a third of respondents prepared or are planning to prepare a birth plan. Primiparas, despite a lower sense of preparation for childbirth as compared with multiparas, valued more the availability of natural than pharmacological measures to reduce the pain, and hoped to receive staff’s help at the first stages of child care, which shows the need for post-natal care and support of women, especially those who gave birth to their first child/children. The multiparas found the first breastfeeding immediately after birth as more important than the primiparas. Our results indicate the importance of treating problems with conceiving for the strength of the bond with the unborn child, and show that significant factors for the strength of bond with the unborn child is a longer contact time with the baby immediately after birth and importance attributed to the access to education and counseling.

Opis:
Polish Red Cross is a humanitarian organization providing aid in the events of natural disasters, mass accidents, and search-and-rescue operations. It educates the citizens by providing first aid training. The Polish Red Cross Recue Group System consists of three types of groups: the Rescue Group, the Medical Rescue Group and the Specialist Rescue Group. They are always ready to provide assistance, especially in times of catastrophes and natural disasters. Moreover, they provide medical support at mass events, such as concerts, live shows, etc.. The aim of this paper was to show in detail how these Rescue Groups function when providing medical support and to describe their legal status. In addition, I have described the organization and functioning of the Polish Red Cross Rescue Team in Bedzin, based on the documentation covering their activity in 2014. The documentation shows that the Rescue Team is a fully functioning, effective and efficient group, always at-the-ready. The number of rescue actions the group participated in shows that its members are fully qualified and experienced, thus the society can rely on them whenever necessary.
Polski Czerwony Krzyż jest organizacją humanitarną, działającą w przypadkach katastrof, wypadków masowych oraz akcji poszukiwawczych. Dodatkowo spełnia rolę edukacyjną, poszerzając wiedzę społeczeństwa na temat udzielania pierwszej pomocy. Wyodrębniony został System Ratownictwa Polskiego Czerwonego Krzyża, w którego skład wchodzą: Grupy Ratownictwa PCK, Grupy Ratownictwa Medycznego PCK oraz Grupy Ratownictwa Specjalnego PCK. Grupy te są w stanie gotowości, aby w sytuacjach krytycznych nieść pomoc społeczeństwu. Dodatkowo zajmują się zabezpieczeniem medycznym imprez o różnym charakterze. Celem pracy jest szczegółowe omówienie działania Grup Ratownictwa podczas zabezpieczenia medycznego oraz przedstawienie jego prawnej organizacji. Dodatkowo przedstawiona została organizacja i funkcjonowanie Grupy Ratownictwa PCK w Będzinie, na podstawie uzyskanej dokumentacji dotyczącej działań podjętych w 2014 roku. Z dokumentacji tej można wnioskować, że grupa ta jest organizacją sprawnie działającą, przygotowaną do szybkiej reakcji w sytuacjach nagłych. Liczba wydarzeń, w których uczestniczyła Grupa Ratownictwa PCK pokazuje, że jej członkowie mają ogromną wiedzę oraz doświadczenie, dzięki czemu społeczeństwo w każdej chwili może liczyć na ich pomoc.

This article deals with problems in functioning of medical support system and social support and integration system addressed to persons with mental disorders in Poland on the example of Greater Poland Voivodeship. This voivodeship is representative as far as the mental health problems’ prevalence and the type of the most common mental disorders diagnosed among people are concerned. The local data reflect very similar tendencies as cross country and international analyses. Moreover, the analysis of the local system of support for people with mental disorders carried out by the authors of this article revealed that the inadequacies of the system reflect the tendencies observed in the cross country Polish scale. In this context, the recommendations formulated by the authors can be applied (with some necessary corrections) in different local environments.
Przedmiotem artykułu jest analiza problemów w funkcjonowaniu systemów wsparcia medycznego i społecznego dla osób z zaburzeniami psychicznymi na przykładzie województwa wielkopolskiego. Dane o zasięgu międzynarodowym, ogólnopolskim i lokalnym wskazują, że województwo wielkopolskie jest reprezentatywne pod względem natężenia oraz typu najczęściej diagnozowanych problemów zdrowia psychicznego ludności. Ponadto analiza systemu wsparcia dla osób doświadczających zaburzeń psychicznych, przeprowadzona przez autorki niniejszego artykułu, wskazuje na reprezentatywność województwa wielkopolskiego w skali kraju pod względem problemów wynikających z niedostatków systemowych rozwiązań mających na celu przeciwdziałanie zaburzeniom psychicznym. Punktem wyjścia w niniejszym opracowaniu jest zarysowanie globalnego kontekstu problematyki zaburzeń psychicznych. Następnie problematyka ta została omówiona w świetle analiz ogólnopolskich i regionalnych. W tym kontekście zostały opisane zagrożenia związane z funkcjonowaniem systemu ochrony zdrowia oraz systemu pomocy i integracji społecznej, a także sformułowano rekomendacje mające na celu przeciwdziałanie barierom rozwoju systemu wsparcia dla osób z zaburzeniami psychicznymi w różnych uwarunkowaniach lokalnych.

Opis:
A complex model of mechanically ventilated ARDS lungs is proposed in the paper. This analogue is based on a combination of four components that describe breathing mechanics: morphology, mechanical properties of surfactant, tissue and chest wall characteristics. Physical-mathematical formulas attained from experimental data have been translated into their electrical equivalents and implemented in MultiSim software. To examine the adequacy of the forward model to the properties and behaviour of mechanically ventilated lungs in patients with ARDS symptoms, several computer simulations have been performed and reported in the paper. Inhomogeneous characteristics observed in the physical properties of ARDS lungs were mapped in a multi-lobe model and the measured outputs were compared with the data from physiological reports. In this way clinicians and scientists can obtain the knowledge on the moment of airway zone reopening/closure expressed as a function of pressure, volume or even time. In the paper, these trends were assessed for inhomogeneous distributions (proper for ARDS) of surfactant properties and airway geometry in consecutive lung lobes. The proposed model enables monitoring of temporal alveolar dynamics in successive lobes as well as those occurring at a higher level of lung structure organization, i.e. in a point P0 which can be used for collection of respiratory data during indirect management of recruitment/de-recruitment processes in ARDS lungs. The complex model and synthetic data generated for various parametrization scenarios make possible prospective studies on designing an indirect mode of alveolar zone management, i.e. with a minimized risk of repeated alveolar recruitment/de-recruitment and mechanical overstraining of lung tissues.

Sleep apnea syndrome is a common sleep disorder. Detection of apnea and differentiation of its type: obstructive (OSA), central (CSA) or mixed is important in the context of treatment methods, however, it typically requires a great deal of technical and human resources. The aim of this research was to propose a quasi-optimal procedure for processing single-channel electroencephalograms (EEG) from overnight recordings, maximizing the accuracy of automatic apnea or hypopnea detection, as well as distinguishing between the OSA and CSA types. The proposed methodology consisted in processing the EEG signals divided into epochs, with the selection of the best methods at the stages of preprocessing, extraction and selection of features, and classification. Normal breathing was unmistakably distinguished from apnea by the k-nearest neighbors (kNN) and an artificial neural network (ANN), and with 99.98% accuracy by the support vector machine (SVM). The average accuracy of multinomial classification was: 82.29%, 83.26%, and 82.25% for the kNN, SVM and ANN, respectively. The sensitivity and precision of OSA and CSA detection ranged from 55 to 66%, and the misclassification cases concerned only the apnea type.

The article has considered the general principles and capabilities of the medical support of NATO troops, confirmed the expediency of further transformation of tasks and functions of the state in terms of medical support of troops (forces) of the security and defense sector of Ukraine in crises through the approximation to the relevant NATO standards, justified the need to implement the Euro-Atlantic experience of rendering medical care, use the general framework of categories and concepts, principles of the evaluation of capabilities (assets) of medical support which will contribute to the development of integrated medical space for medical defense of troops (forces) and elaboration of individual crisis response model taking into account the basic international requirements. The authors have determined the priority tasks of further cooperation between Ukraine and the North Atlantic Treaty Organization in the context of the reforms of medical logistics. It has been substantiated the need to keep reforming military medicine in conformity with NATO standards and recommendations that will promote an active involvement of medical services of troops (forces) in medical support of national and multi-domestic contingents during crises and adaptation of interoperable medical capabilities of joint missions under the auspices of NATO and the UN. The research has analyzed the mechanisms of interdepartmental coordination on medical support of troops (forces) and proved the feasibility of using the experience of NATO member states in the organization and functioning of the system of medical support of the military contingent with the adaptation of NATO doctrinal documents to the realities of modern Ukraine.

In medical decision making (e.g., classification) we expect that decision will be made effectively and reliably. Decision making systems with their ability to learn automatically seem to be very appropriate for performing such tasks. Decision trees provide high classification accuracy with simple representation of gathered knowledge. Those advantages cause that decision trees have been widely used in different areas of medical decision making. In this paper we present characteristic of univariate and multivariate decision tree. We apply those classifiers to the problem of acute abdominal pain diagnosis.

The paper presents a study on data-driven diagnostic rules, which are easy to interpret by human experts. To this end, the Dempster-Shafer theory extended for fuzzy focal elements is used. Premises of the rules (fuzzy focal elements) are provided by membership functions which shapes are changing according to input symptoms. The main aim of the present study is to evaluate common membership function shapes and to introduce a rule elimination algorithm. Proposed methods are first illustrated with the popular Iris data set. Next experiments with five medical benchmark databases are performed. Results of the experiments show that various membership function shapes provide different inference efficiency but the extracted rule sets are close to each other. Thus indications for determining rules with possible heuristic interpretation can be formulated.

Paper deals with the knowledge acquisition process. Different experts formulate the rules for decision support systems. We assume they have different knowledge about the problem and therefore obtained rules have different qualities. We will formulate the proposition of the confidence measure and its application to the decision process. We will propose how calculate the value of measure under consideration for typical statistical learning process. On the base on the proposed measure of the knowledge quality we propose the procedure of the contradictions elimination for the set of logical rules.
