Opis:
Chronic condition may, in time, become a specific kind of „stigma” that determines the formation of a specific “conveyor belt” which determines the trajectories of the fate of children and adolescents. Analysing the functioning of children and adolescents with a chronic condition, for which the school should provide (as for other students) the conditions of learning, both in the cognitive and in the directional aspects, one may indicate not only the imperfections of educational activities, but also a myth of noble ideas inscribed in the function of school, as a place of universal development of every student. The article is a proposal of employing, within the process of educating children and adolescents with a chronic condition, an eclectic model of activities, based on their complexity and sequentiality. The model refers to models proposed by Frederick C. Thorne, or Richard James, used in situations of crisis intervention: the equilibrium model, the cognitive model, and the social change model.

Opis:
Both the beginning and the end of human life are connected to many moral dilemmas. The discussion becomes especially emotional when we deal with the situation of parents (or sometimes just a mother) who decide about their baby’s life in its prenatal phase of development. The in vitro procedure itself still gives rise to a number of doubts in some circles. Especially the multiple embryo transfer exposes the mother to a potential stress of deciding about the further course of pregnancy. To be more precise: some patients are persuaded into selective abortion. Polish law allows abortion in three cases. Still, the act of 1997 does not seem to be precise enough. On the one hand there is a threat of it being abused, on the other – there are difficulties when it comes to it being executed. Due to the medical advancement issues that were obvious twenty years ago need to be newly evaluated (e.g. the age of an infant capable of surviving outside the mother’s womb is gradually decreasing). It also seems that it is necessary to look upon terms such as abortion, termination, lethal defects, overzealous therapy, abandoning overzealous therapy, euthanasia (active, passive), euthanasia attitude, hospice care attitude, chronic condition, terminal phase, terminal state, palliative care. It becomes crucial especially now when Poland is faced with projects of opposing legal acts: those delegalizing abortion completely and those allowing for complete freedom of it. Therefore, a sound bioethical discussion is in order, such that will allow for the optimal standpoint. At the same time it would be beneficial to concentrate on thanalogy education because, as distant and unrealistic those ultimate matter may seem now, they concern every human being.

Atopic eczema (AE) is a multifactorial skin disease caused by a variety of factors such as genetic conditions, alterated skin structure, immunologic deviations and environmental factors, among others. There are two main subtypes of AE, i.e. the IgE-associated (“atopic eczema”) and the non-IgE-associated type (“nonatopic eczema”) with different prognosis concerning the development of respiratory diseases (“atopy march”). Recently, it was demonstrated that Filaggrin (= fi lament-aggregating protein, FL) is a major gene for atopic eczema. Filaggrin binds to and aggregates the keratin cytoskeleton in the epidermis. Homozygous FLG mutation leads to complete loss of fi laggrin expression in skin. Half or more of children with moderate to severe AE carry FLG mutations. Moreover, fi laggrin loss-of-function mutations predispose to phenotypes involved in the atopy march. The altered skin structure and a defi ciency in antimicrobial peptides favour colonization with Staphylococcus aureus and yeasts (Malassezia sp.). Sensitization to the yeast occurs almost exclusively in AE patients. S. aureus enterotoxins with superantigenic activity stimulate activation of T cells and macrophages. So far, AE skin lesions are orchestrated by the local tissue expression of proinfl ammatory cytokines and chemokines with activation of T lymphocytes, dendritic cells, macrophages, keratinocytes, mast cells, and eosinophils which lead to the skin infl ammatory responses. From the therapeutic point of view, besides emollients and local corticosteroids, topic immunomodulatory drugs (tacrolimus and pimecrolimus) have substantially improved the treatment of AE.
