Introduction: Malaria is the most common tropical infectious disease caused by parasitic protozoa, affecting every year about 220 million of people with about 1-3 million deaths. It is known that every 30 seconds one African child dies from malaria. Moreover, malaria as the most common imported tropical disease is the most frequent cause of deaths. Currently, in African countries, every year, about 300-500 million people have malaria, and more than a million die from malaria. A high percentage of deaths refers to children under 5 years old, pregnant women and susceptible individuals. Materials and methods: The examination was carried out from July to September, 2011 year in 60 peoplepatients of the Out-patients Clinic f St. Francis Hospital, in Kampala, the capital of Uganda, using the questionnaire created by author.Results: As many as 96.7% of respondents answered that the best method was avoiding mosquitoes via using bed mosquito nets and protective clothing. A total of 51.7% indicated chemical substances as an effective protective method against malaria. The surveyed named repellents such as liquids, aerosols, creams and sticks. Thirty two people (53.7%) wrote that an effective protective method against malaria was to avoid mosquitoes’ incubation sites, to remove garbage frequently and to clear regularly ponds round the houses.Conclusion: Ugandan patients identified the risk factors and groups, non-key symptoms and prophylaxis against malaria. The majority of Ugandan patients indicated there was vaccine against malaria. More than half of Ugandan patients (65%) declared that a mosquito bite is the route of malaria infection, however, more than 50% of those living in rural areas thought on other routes such as, e.g. unboiled water.

Malaria diagnosis continues to be one of the most important steps in the cycle of control specially in endemic countries with low parasitic load infections. Loop-mediated isothermal amplification (LAMP) and ultrasensitive PCR (Us-PCR) are two promising candidates for malaria diagnosis. A cross sectional study performed at King Faisal Hospital, Taif KSA involved patients suffering from signs and symptoms suggesting of malaria, 35 blood samples diagnosed by Nested Multiplex PCR as a reference method (13 P. falciparum, 17 P. vivax, 3 mixed P. falciparum and P. vivax) plus two negative controls were selected to be included in this study to analyse the performance of two LAMP methods (LAMP OptiGene® and LAMP WarmStart®) and two ultrasensitive PCRs (Us-PCR TARE-2 and Us-PCR Var-ATS). LAMP OptiGene® and LAMP WarmStart® performances were identical and better than the performance of Us PCR TARE 2 and Us-PCR var-ATS for P. falciparum, achieving 93.75% sensitivity, 100% specificity and 97.17% accuracy versus 87.5% sensitivity, 100% specificity and 94.29% accuracy for the Us PCR TARE 2 and 81.25% sensitivity, 94.74% specificity and 88.57% accuracy for the Us PCR var-ATS respectively. In P. vivax diagnosis LAMP OptiGene® performed excellently with 100% sensitivity, specificity, and accuracy while LAMP WarmStart® and Us-PCR Cox1 achieved 100% sensitivity, specificity 93.33% and 97.14% accuracy. The study results highlighted the benefits of using LAMP techniques for field diagnosis of malaria in different settings where the need for a more sensitive and reliable molecular tool is mandatory but at the same time removing the high cost, long turnaround time and the need of highly specialized trained technicians to perform more sophisticated molecular techniques.

The high prevalence of malaria in Africa has defiled many strategies aimed at its eradication. Researchers from various fields have tried without success in this fight against mosquito and its malaria disease. Annually billions of dollars are spent in the design of programs which are aimed at combating this dreaded disease. However all this spending seems to go down the drain as malaria and its vector mosquitoes celebrate their unflinching victory. Current control measures focusing on ways of preventing the disease vis- a -vis, protect man from the vectors “anopheline mosquito” are the mainstay of malaria prevention and control. Many of these control measures are operational with each contributing in its little way. The use of Long Lasting Insecticide Treated Nets (LLITN) and Indoor Residual Sprays (IRS) are well established strategies with global recognition and currently ongoing in Africa. However, as a result of shortcomings in these major control measures, new strategies with hopes of blissful success are been sought after. Larviciding (abortion of metamorphosis) and constant and adequate environmental sanitation seems to be the next option available for use. This article therefore takes a look at the vector- anopheline mosquito, its ecology, productivity and distribution. It also considers malaria and the various control and preventive measures currently targeted at its eradication.

Introduction and aim. Accurate identification of Plasmodium species is important because of the differences in their treatment. We aimed to investigate the role of hematological and biochemical parameters in the differentiation of Plasmodium falciparum and other plasmodium species. Material and methods. This is a retrospective study. Patients admitted to the emergency department with signs and symptoms of malaria were included into the study. Patients with malaria were grouped as P. falciparum and others. Hematological parameters of two groups were compared by univariate and multivariate analysis. Statistical analysis was performed using the Jamovi. Results. A total of 107 patients were included in the study. According to univariant and multivariant analysis there was no difference in between two groups in the terms of blood urea nitrogen, aspartate aminotransferase, total bilirubin, hemoglobin, hematocrit, white blood cell count, platelet count, and mean platelet volume (in univariate analysis p values were 0.029, 0.011, 0.019, 0.171, 0.870, 0.307, 0.042, and 0.276, respectively and in multivariate analysis p values for blood urea nitrogen, aspartate aminotransferase, total bilirubin, hemoglobin, and platelet count were 0.100, 0.535, 0.328, and 0.213, respectively). Conclusion. The investigated hematological and biochemical parameters were found to be not valuable in predicting type of malaria. On the other hand, we recommend confirming the results of our study with larger samples and multicenter studies.

Whether maternal peripheral parasites constitute a representative sample of the overall population infecting the individual, remains unknown in Burkina Faso. We therefore compared Pfdhfr and Pfdhps genotypes between matched peripheral and placental isolates. PCR-restriction fragment length polymorphism (PCR-RFLP) analysis of polymorphic codons of the Pfdhfr gene (51, 59, 108 and 164) and the Pfdhps gene (437 and 540) was performed in 18 matched peripheral and placental dried blood spots of delivered women in Bobo-Dioulasso. Both Pfdhfr and Pfdhps genes were successfully genotyped in 94.4% (17/18) of the matched samples. Only 8.8% (3/34) of genotypes were of the wild type, while 20.6% (7/34), 20.6% (7/34), 23.5% (8/34) and 26.5% (9/34) comprised one, two, three and four mutations, respectively. None of the samples carried both Pfdhfr I164L and Pfdhps K540E mutations. A concordance of 82.4% was observed in matched samples for both the Pfdhfr and Pfdhps genes. Setting placental alleles as the reference, a concordance of 100% was obtained with Pfdhfr mutation S108N, Pfdhfr mutation C59R+S108N, and Pfdhfr mutation N51I+C59R +S108N, respectively. Likewise, a concordance of 85.7% was observed with the Pfdhps mutation A437G. For epidemiological purposes, peripheral blood Pfdhfr and Pfdhps genotyping is sufficient for monitoring SP resistant molecular markers in pregnant women.

Satoshi Omura, mikrobiolog z Japonii i William Campbell amerykański biochemik i parazytolog, pochodzący z Irlandii, zostali nagrodzeni za zidentyfikowanie i wyizolowanie awermektyny ze szczepu bakterii Stretomyces avermicilis produkującego związek o silnej aktywności przeciwnicieniowej. Zastosowanie artemizyniny i iwermektyny ocaliło życie milionów dzieci w Afryce oraz zahamowało szerzenie się filarioz, groźnych chorób przyczyniających się do utraty wzroku i deformujących ciało ludzkie. W artykule omówiono historię odkrycia i mechanizm działania leków. Wprowadzenie globalnych programów leczenia w krajach tropikalnych przyczynia się do poprawy sytuacji ekonomicznej i społecznej na obszarach, na których choroby te dzięki lekom są coraz rzadziej notowane.
The Nobel Prize in Physiology or Medicine 2015 was awarded to Youyou Tu "for her discoveries concerning a novel therapy against Malaria", and William C. Campbell and Satoshi Ōmura "for their discoveries concerning a novel therapy against infections caused by roundworm parasites". Chinese pharmacologist - Tu, identified artemisinin isolated from Artemisia annuata for inhibition of malaria development in the blood of man. Omura, microbiologist from Japan and William C. Campbell, biochemist and parasitologist from USA discovered anti-nematode activity of awermectin, extracted from bacteria Streptomyces avermicilis. Administration of drugs against malaria and filariasis salvaged millions lives, and reduced transmission of pathogens responsible for the lost vision and disability in Africa, Asia and Latin America. The history of the discoveries and mechanisms of drugs action are presented. International programs for the health introduced at the global scale have improved economic and social conditions in countries where diseases due to the treatment develop less frequently.

In Sahelian countries such as Burkina Faso, malaria transmission is seasonal with a high incidence of transmission during the rainy season. This study aimed to compare the effectiveness of the two recommended treatments (Artemether-Lumefantrine and Artesunate-Amodiaquine) for uncomplicated malaria in Burkina Faso regarding this seasonal variation of malaria transmission. This is part of a randomized open label trial comparing the effectiveness and safety of Artemether-Lumefantrine versus Artesunate-Amodiaquine according to routine practice in Nanoro. Patients with uncomplicated falciparum malaria were recruited all year round and followed-up for 28 days. To distinguish recrudescences from new infections, dried blood spots from day 0 and day of recurrent parasitaemia were used for nested-PCR genotyping of the polymorphic loci of the merozoite surface proteins 1 and 2. Seasonal influence was investigated by assessing the treatment outcomes according to the recruitment period of the patients. Two main groups (dry season versus rainy season) were defined following the seasonal characteristics of the study area. In Artemether- Lumefantrine group, the uncorrected cure rate was 76.5% in dry season versus 37.9% in rainy season. In Artesunate-Amodiaquine group, this was 93.3% and 57.1% during dry and rainy seasons, respectively. After PCR adjustment, the cure rate decreased from 85.9% in dry season to 75.0% in rainy season in Artemether-Lumefantrine group. In Artesunate-Amodiaquine group, it was 93.3% in dry season and 80.7% during the rainy season. During the rainy season around 50% of patients had a new malaria episode by Day 28. The cure rate of both Artemether-Lumefantrine and Artesunate-Amodiaquine treatments was higher in dry season compared to rainy season due to high incidence of reinfections during the rainy season. For this reason, in addition to the curative effect, the post-treatment prophylactic effect should be taken into account in the choice of antimalarial regimens.

Objectives This study analyzed the effect of the number of mosquito nets that are owned by households, dwelling characteristics and maternal demographic characteristics on malaria infections. Material and Methods The 2011 Demographic and Health Survey (DHS) data for children under 5 years of age were used. The children were subjected to haemoglobin test and rapid diagnostic test (RDT) to ascertain the presence of malaria parasites. Data were analyzed using probit regression method. Results It was found that 2.43% and 8.68% of the children were living in houses that were prone to landslide and flooding, respectively. Also, 19.93%, 17.08% and 16.26% of the children lived in houses without windows, with broken windows, and with a hole in the roof, respectively. Only 5.59% and 23.96% of the children lived in houses with window and door nets, respectively. Mosquito nets were owned by 64.03% of the households, where Adamawa Region had the lowest coverage (52.23%). Reasons for not owning mosquito nets by all the households included: lack of financial means (25.17%), using something else (1.80%) and not having many mosquitoes in the vicinity (5.53%). In the probit regression, variables that significantly reduced malaria infections among the children (p < 0.05) included: the number of mosquito nets, urban residence, improved toilet, ownership of a radio, residence in flood-prone area, mother’s secondary education, mother’s tertiary education and residence in areas with not many mosquitoes, while infections increased along with the household size, residence in areas prone to landslide, severe anaemia, moderate anaemia, mild anaemia and age of the children. Conclusions Ownership of mosquito nets and dwelling characteristics are critical factors influencing infections with malaria. There is a need to ensure compliance with its use since there are disparities between access and actual usage. Also, addressing malaria problem in Cameroon should consider regional disparity in malaria incidence rates and more engagement of the media, among others, for appropriate sensitization.

Opis:
Objective: To study the glycaemic profile of patients with severe malaria (SM). Methods: For this purpose, 110 SM patients were recruited. Pre-treatment random blood glucose and plasma insulin were measured in a subset of donors. An ex-vivo experiment was developed for estimation of glucose consumption by parasitized erythrocytes. Results: Hyperglycaemia was frequent in SM but more commonly associated with cerebral malaria (CM), while hyperinsulinaemia was recognized in severe-malarial-hypotension (median, 25 %-75 %, 188.2, 93.8-336.8 pmol/L). The plasma insulin level was positively correlated with age (CC = 0.457, p < 0.001) and negatively with parasitaemia (CC = -0.368, p = 0.045). Importantly, fatal-CM was associated with hyperglycaemia (12.22, 6.5-14.6 mmol/L), hyperinsulinaemia (141.0, 54.0-186.8 pmol/L) and elevated homeostasis model assessment (HOMA) values. However, there was a trend of higher glucose consumption by parasites in CM compared with that in uncomplicated malaria (UM). Conclusion: Hyperglycaemia, hyperinsulinaemia and elevated HOMA are evidence for insulin resistance and possibly pancreatic B-cell dysfunction in fatal-CM.
