This paper presents two FE simulations of an oncology knee-joint endoprosthesis. Firstly dynamic experiment-based analysis traditionally made on special knee simulator has been calculated. Another FE simulation of an oncology knee-joint endoprosthesis was made on a complex model based on previously developed geometric model of a lower limb. The presented paper aims to prove the potential of finite element method in biomechanics, especially in development of joint endoprosthesis.

Introduction and aim. Neutropenic enterocolitis is a severe disease that affects patients with immunodeficiency and is often related with the chemotherapy of the hematologic malignancies. Its pathophysiology is not fully understood. Neutropenic colitis may lead to sepsis, gastrointestinal bleeding and even perforations requiring surgical management. The therapy consists of antibiotic therapy, transfusions, hematopoietic growth factor treatment, usage of fluids and electrolytes, bowel rest and even surgical operations. The aim of this review is to consider the potential usage of probiotics in the prevention and treatment of neutropenic enterocolitis. Material and methods. References for that article were found through PubMed and Google Scholar, using terms: “neutropenic enterocolitis” and “probiotics”, or “gut microbiota” and “neutropenic enterocolitis”. The research was limited to abstracts and available full-text articles. Analysis of the literature. The most possible mechanism of neutropenic enterocolitis development appoints bacterial invasion with co-existing immunodeficiency. The probiotics appeal as beneficial agents in both prevention and treatment of neutropenic enterocolitis in according to their impact on gut immune barrier improvement. However older societies’ guidelines were cautious, the most modern ones appoint probiotics as a promising agent in neutropenic enterocolitis, what corresponds with results from current randomized clinical trials. Conclusion. As neutropenic enterocolitis is a severe disease we need to look for better or alternative therapies of that state. The probiotics seems to have beneficial effects in terms of prevention and treatment of neutropenic enterocolitis due to their impact on gut immune barrier. Benefits of such therapy are reflected in current societies’ guidelines which consider probiotics as a promising agent in neutropenic enterocolitis.

Nanotechnology is an interdisciplinary area of science devoted to the production and testing of nanostructures - defined as forms of the matter organizations the size of which does not exceed 100 nm. It is a quickly developing area of science with many applications in different areas of life, for example in engineering, computing, medicine, pharmacy, andagriculture. One of the problems of contemporary oncology is the low specificity of applied therapies. Most currently used chemiopharmaceuticals have systemic effects which not only affect cancer cells but alsohealthy tissues. Complications after chemotherapy observed in many patients are bone marrow deficiency(neutropenin, thrombocytopenia, anemia), damageto the nervous system (neurotoxicity), myocardium(cardiotoxicity) and pulmonary parenchyma. Similarly, in radiotherapy, ionizing radiation destroys the healthy tissues in the irradiation field. The side effects of radiation therapy may include fatigue, skin reactions, and impairment of tissue and organ functions. According to studies, nanostructures are an opportunity to overcome these limitations. The most popular nanostructures used in medicine are liposomes, silver and gold nanoparticles, magnetic nanoparticles, carbonnanotubes, and dendrimers. The purpose of this article is to present the current state of knowledge on the use of available nanotechnology solutions in pharmacology and cancer treatment.

Cancer causes huge problems, physical and mental nature mainly. In particular, we cannot forget about the functioning of these patients in the social and spiritual spheres. The increasing trend of incidence of rectal cancer makes the disease is becoming a priority for doctors, nurses and psycho-oncologists. Despite the increasing quality of medical services, patients face a number of problems associated with cancer treatment, which may result in formation of a colostomy. The procedure, which is necessary to save the patient’s life, is often perceived by them as “mutilation”. Acceptance of the disease and satisfaction of life in patients with a stoma after the operation for rectal cancer are dependent on many factors. Social support, living conditions and the time elapsed since creation of the stoma have great impact. “Stoma nurse” plays an extremely important role. Seeing the difficulties in adapting the stoma she should verify the patient’s pessimism as to his/her own self, develops a sense of responsibility from minor to major issues and strengthen a sense of independence.

Koordynowana opieka nad pacjentem stała się jednym z głównych tematów rozważań nad zmianami w polskim systemie ochrony zdrowia. Doświadczenia zaczerpnięte z praktyk międzynarodowych pozwoliły na stworzenie pewnej podstawy zarządzania procesem leczenia, jednak w dalszym ciągu widoczna jest silna potrzeba wprowadzania zmian. W artykule został poddany analizie sposób zarządzania pacjentem onkologicznym w ramach istniejącego pakietu onkologicznego. Przedstawiono założenia rozwiązania systemu kolejkowego oraz poddano ocenie skutki rozwiązań w polskiej onkologii. Z przeprowadzonej analizy wynika, że trudno jest wnioskować o efektywności kosztowej i klinicznej pakietu onkologicznego w ramach koordynacji opieki nad pacjentem z powodu braku istotnych danych w momencie jego wdrożenia. Poddany konstruktywnej krytyce pakiet onkologiczny obecnie jest w fazie zmian. Pozostaje postawić pytanie, na ile kryteria nowelizacji reformy są istotne z punktu widzenia klinicznego, społecznego i płatnika. Nie zapominajmy o ukierunkowaniu opieki koordynowanej, której celem w przypadku chorób nowotworowych jest szybka diagnostyka i celowane leczenie.
Coordinated patient care (disease management) has become one of the main topics of discussion in the Polish health care system. The experience drawn from international practices helped to create a basis for managing the process of treatment. Nevertheless, there is still a strong need for changes in the Polish healthcare. The article presents the management of oncological patients within the existing Package of Oncology. The assumptions of this consideration involve the analysis of oncology queue system solutions and their impact on the patients care. The analysis shows that it is difficult to conclude about the effectiveness and cost of the clinical oncology package within the framework of coordination of patient care due to the lack of relevant data at the time of its implementation. Having been subjected to constructive criticism, the oncology package is currently in the phase of changes. The question remains to what extent the criteria for amendment of the reform are important from clinical, social and the payer’s point of view. Moreover, one should not forget about the main assumptions of coordinated cancer care, which are: immediate diagnosis and fast introduction of effective treatment.

Ghrelin is a 28 amino-acid multi-functional peptide hormone, which was identified as a natural ligand of the growth hormone secretagogue receptor (GHS-R). Pituitary growth hormone-releasing activity in both animals and humans has been well documented. It has various biological functions, including regulation of appetite and body weight, control of energy homeostasis, modulation of cardiovascular and gastrointestinal system and anti-inflammatory effect. However, both ghrelin and its receptor (GHS-R) are widely distributed in various tumors, which strongly implies their role in neoplastic cell growth trough autocrine/paracrine mechanism. Multiple studies have demonstrated the role of ghrelin in cancer cells proliferation, differentiation, invasiveness and apoptosis inhibition. The ghrelin axis is more complex than it was originally thought and consist of several compounds that might interact with each other and affect ghrelin activities. Here, we provide an overview of the ghrelin and its receptor role in tumor progression.

Anticancer therapies have extended the lives of patients with cancer, but for some, this benefit is tempered by cardiovascular complications. Their number is increasing as a result of an improvement in the early diagnosis of cardiotoxicity caused by chemo- and radiotherapy. Therefore prevention, detection, monitoring, and treatment of cancer patients at risk of cardiotoxicity with or without concomitant CV diseases are very important. Cardio-oncology is a new direction in Ukraine for improving clinical outcomes of cancer patients. This review aims to provide an overview of the rationale for setting up a Cardio-Oncology Unit and reflects our own experience establishing this service.

Problem identification: The desire for motherhood is one of the top priorities for most female young cancer survivors. The risk of infertility after cancer therapy is an important concern with several physical and emotional consequences. This comprehensive literature review aims to summarize and contextualize recent research that has been carried out on female fertility after cancer, suggesting future research and clinical directions. Literature search: Searches included the keywords “cancer”, “female”, “adults”, and “fertility”. Studies were selected if focused on infertility issues in female cancer survivors. Data synthesis: Eight themes were identified: the effects of anticancer treatments on fertility; fertility preservation methods; international recommendations regarding infertility risk; health professionals’ attitudes towards fertility preservation; patient’s concerns regarding the risk of infertility; patients' information needs; and the impact of the risk of infertility in patient’s quality of life. Conclusions: Psycho-oncologists should be integrated in the oncofertility teams to assess patients’ needs before cancer treatment initiation.

Nonbacterial thrombotic endocarditis is a potentially overlooked condition that involves the formation of sterile fibrin vegetations on heart valve leaflets in cancer patients. This condition can lead to valvular dysfunction, heart failure and systemic embolization. The pathogenesis depends on hypercoagulable states which are common in malignancy scenarios. Diagnosis requires the presence of valvular vegetations and the absence of infection.

Geriatric patients with cancer are very ununiform group with their biological age, comorbidities and conditions. Introduction and aim. The incidence of cancer is increasing on a daily basis. One of the methods used for treatment is radiotherapy. Owing to interventions during the radiotherapy process, the patient may experience care dependency. In this study, the aim was to investigate care dependence and related factors in radiation oncology patients. Material and methods. This was a descriptive and cross-sectional study. Data were collected between September 2020 and September 2021. In the collection of data, a sociodemographic information form and a Care Dependency Scale were used. The sample consisted of 52 people. Results. Number of participants was 52, mean age was 60.25±11.715, mean care dependency score (initial) 66.19±18.966, mean care addiction score (final) 66.27±22.795. Conclusion. The care dependency of patients hospitalized in the radiation oncology clinic is moderate. The care dependency of these patients decreased partially during their stay in the clinic. The patient’s inability to walk, speak and the presence of a companion affected the patient’s condition. By evaluating the care dependency levels of the patients, the awareness of the nurses about their patients can be increased. In addition, it may be appropriate to consider the care dependency levels of the patients for the nurse workforce planning to work in the oncology clinic.

For many speech therapists, regardless of their work experience, undertaking therapy with a patient after complete removal of the larynx is a huge challenge that they often cannot cope with. This state of affairs is caused by the still small number of practical studies containing detailed guidelines for post-laryngectomy therapy. The article presents detailed recommendations on evoking esophageal voice along with an explanation of the causes of possible failures during the voice and speech rehabilitation process. Possible actions range from those that are possible a few days after laryngeal surgery to those for practicing the prosodic elements of speech.
Dla wielu logopedów, niezależnie od stażu pracy, podjęcie terapii z pacjentem po całkowitym usunięciu krtani stanowi ogromne wyzwanie, któremu nierzadko nie umieją sprostać. Taki stan rzeczy jest spowodowany wciąż niewielką liczbą praktycznych opracowań zawierających szczegółowe wskazówki dotyczące terapii po laryngektomii. W artykule przedstawiono szczegółowe zalecenia dotyczące terapii mowy przełykowej wraz z objaśnieniem przyczyn możliwych niepowodzeń podczas procesu rehabilitacji głosu i mowy. Omówiono możliwe działania począwszy od kilku dób od operacji usunięcia krtani aż do momentu trenowania prozodycznych elementów mowy.

Acute coronary syndrome (ACS) and oncological disease are more frequently observed in the general population as discussed in the part I of this article. Treatment of myocardial infarction in oncological patients becomes a real struggle for clinicians, especially that the data from clinical trials including cancer patients with ACS are very limited. The choice of treatment modality should consider many existing factors considering the type of ACS – non-ST-segment elevation myocardial infarction vs ST-segment elevation myocardial infarction, patient’s condition, type of cancer and oncological treatment applied. Taking into consideration above mentioned factors, clinicians have to face three therapeutic options: invasive, conservative or combination of both in order to choose the best and most beneficial treatment. This article summarizes the current therapeutic approach to the management of ACS in cancer patients.

The aim of the study was an evaluation of conversion causes and intraoperative difficulties in patients treated with laparoscopic surgery due to colorectal tumours on the basis of authors' own material.Material and methods. in the period 2009-2012 160 patients were operated on because of large bowel cancer, 79 women and 81 men. Average age of patients was 66 years. The conversion was recognized as desuflation of peritoneal cavity, trocars removal and making laparotomy.Results. The conversion was made in 22 cases (13,7%). More in men (16.1%) than in women. The conversion was made twice as likely in T3 and T4 tumors than in T1 and T2.Conclusions. The main cause of conversion in laparoscopic operations of colorectal cancer is local tumor progression.

Cancer is a very important problem in both medical and social respects. In recent years, there has been a shift in approach to oncological prevention, diagnostics and treatment. As a result, oncology has become an interdisciplinary field. Factors relevant for successful oncological treatment are: sequence, type and scope of intervention, including diagnosis of the cancer, assessment of its progression, systemic treatment, surgery, radiotherapy, supportive therapy and rehabilitation. Rehabilitation addresses the realm of psychology (psychooncology), as well as somatic and social issues. Physical therapy is a notion inseparably related to medical rehabilitation and it encompasses a range of treatments which are based on the body’s reactivity to stimuli. The purpose of this article is to present the specific character, the methods and the role of physical therapy in oncology, as an increasingly popular strategy in medicine, which helps to improve performance and physical function in cancer patients. Physical therapy is essential in primary and secondary cancer prevention and it greatly contributes to improving the quality of life of patients and helps them recover quicker. Four basic kinds of intervention in oncological rehabilitation include: preventive interventions, restorative interventions, supportive interventions and palliative interventions. The main principle in rehabilitating a patient with an advanced cancer is progressing steadily but gradually. The primary and essential form of rehabilitation for cancer patients is movement exercise i.e. kinesiotherapy. Integration and cooperation during group exercise are also among the strategies that therapists seek to employ while working to improve the condition of patients diagnosed with cancer. Kinesiotherapy prevents pulmonary and thromboembolic complications in cancer patients. One method of physical therapy applied in cancer treatment is lymphatic drainage (massage), which improves circulation of lymph. To sum up, physical therapy plays an increasingly important role in holistic care of cancer patients. It is indispensable and should become a standard approach, as a method of reducing the risk of complications, helping in faster recovery and limiting the economic and social costs of treatment.
