Critical moments during gymnastic exercises are those related to landing and suppressing overloads, transmitted through lower limbs. A swimmer’s lower limbs provide a good body position in the water and maintain the balance. Both in sports authors postulate reaching bilateral, steady development of the athlete’s body. The main aim was identification and a comparative analysis of the hip, knee and ankle joints flexors and extensors strength, of highly qualified gymnasts and swimmers. Ten healthy subjects participated in the investigation. Five gymnasts (22.2 ±7.3 y.o., 169.4 ±3.91 cm, 64.4 ±3.78 kg) and five swimmers (20.5 ±1.3 y.o., 186.2 ±1.64 cm, 78.4 ±2.61 kg) were highly qualified Polish athletes. Strength measurements were made on Biodex S4 isokinetic dynamometer. Subjects performed hip extension/flexion in lying position, knee extension/flexion and ankle plantar/dorsi flexion. Strength parameters of the lower limbs differentiate gymnasts and swimmers. Lower limb’s strength parameters can be used during training, control, selection or prevention. To achieve a high sport level, swimmers must demonstrate higher strength parameters of lower limbs than gymnasts. The value of work in the maximum repetition (MRW) should be thoroughly analyzed. Highly qualified gymnasts and swimmers should take into account the harmonious, bilateral strength development of the lower limbs.

This paper presents the process of development, as well as examples of devices and systems supporting rehabilitation of the human lower extremities, developed independently over the years in many parts of the world. Particular emphasis was placed on indicating, which major groups of devices supporting kinesitherapy of the lower limbs can be distinguished, what are the important advantages and disadvantages of particular types of solutions, as well as what directions currently dominating in development of rehabilitation systems may be specified. A deeper analysis and comparison of several selected systems was also conducted, resulting in gathering the outcomes in two tables. They focused on a few features of mechanical design, especially the devices’ kinematic structures, and devices’ additional functions associated with, among others, interaction, as well as diagnosis of the limb's state and the progress of rehabilitation.

Opis:
Purpose: Based on the analysis of existing solutions, biomechanics of human lower limbs and anticipated applications, results of considerations concerning the necessary number of degrees of freedom for the designed device supporting rehabilitation of lower extremities are presented. An analysis was carried out in order to determine the innovative kinematic structure of the device, ensuring sufficient mobility and functionality while minimizing the number of degrees of freedom. Methods: With the aid of appropriate formalised methods, for instance, type synthesis, a complete variety of solutions for leg joints were obtained in the form of basic and kinematic schemes, having the potential to find application in devices supporting lower limb rehabilitation. Results: A 3D model of ankle joint module was built in Autodesk Inventor System, then imported to Adams and assembled into a moving numerical model of a mechanism. Several conducted simulations resulted in finding the required maximum stroke of the cylinders. Conclusions: A comparison of the angular ranges of ankle joint and similar devices with the ones achieved by the designed device indicated a sufficient reserve allowing not only movements typical of gait, but approximately achieving the passive range of motion for the ankle joint.

Purpose: Nordic Walking (NW) is a sport that has a number of benefits as a rehabilitation method. It is performed with specially designed poles and has been often recommended as a physical activity that helps reduce the load to limbs. However, some studies have suggested that these findings might be erroneous. Study aim. The aim of this paper was to compare the kinematic, kinetic and dynamic parameters of lower limbs between Natural Walking (W) and Nordic Walking (NW) at both low and high walking speeds. Methods: The study used a registration system, BTS Smart software and Kistler platform. Eleven subjects walked along a 15-metre path at low (below 2 m⋅s–1) and high (over 2 m⋅s–1) walking speeds. The Davis model was employed for calculations of kinematic, kinetic and dynamic parameters of lower limbs. Results: With constant speed, the support given by Nordic Walking poles does not make the stroke longer and there is no change in pelvic rotation either. The only change observed was much bigger pelvic anteversion in the sagittal plane during fast NW. There were no changes in forces, power and muscle torques in lower limbs. Conclusions: The study found no differences in kinematic, kinetic and dynamic parameters between Natural Walking (W) and Nordic Walking (NW). Higher speeds generate greater ground reaction forces and muscle torques in lower limbs. Gait parameters depend on walking speed rather than on walking style.

Purpose: The aim of the study was to determine the relationships between time of running over a 15–25 m section of a 30-meter run along a straight line and changes in the angle and angular velocity observed in ankle, knee and hip joints. Therefore, the authors attempted to answer the question of whether a technique of lower limbs movement during the phase of sprint maximum velocity significantly correlates with the time of running over this section. Methods: A group of 14 young people from the Lower Silesia Voivodeship Team participated in the experiment. A Fusion Smart Speed System was employed for running time measurements. The kinematic data were recorded using Noraxon MyoMotion system. Results: There were observed statistically significant relationships between sprint time over a section from 15 to 25 m and left hip rotation (positive) and between this time and left and right ankle joint dorsi-plantar flexion (negative). Conclusions: During the maximum velocity phase of a 30 m sprint, the effect of dorsi-plantar flexion performed in the whole range of motion was found to be beneficial. This can be attributed to the use of elastic energy released in the stride cycle. Further, hip rotation should be minimized, which makes the stride aligned more along a line of running (a straight line) instead of from side to side.

Purpose: The aim of this study was to determine how minimalist running shoes (MRS), conventional running shoes (CRS) and the dominance of lower limbs influence the running gait. Methods: Trained recreational runners (N = 13) who have been engaged in regulary running for more than one year were participants in this study. They were experienced with using MRS and CRS for more than half year and they used both types of shoes. An in-shoe pressure measuring system (Pedar-X®, Novel, Munich, Germany) was used to monitor plantar pressure and vertical force and the temporal parameters when running in MRS and CRS during the stance phase, the swing phase and over one stride. Results: Running in CRS significantly prolonged stance, swing and stride phases by 2–11% compared to MRS. In contrast, when running in MRS significantly larger values of maximum pressure (9–14%) and maximum vertical force (3–7%) than in CRS were found. Conclusions: For this reason, running in MRS could be recommended to recreational runners only with care. The effect of limb dominance on temporal characteristics was detected when running in CRS. Significantly longer stance phase for dominant limb is associated with a shorter swing. The kinematics variables were significantly higher for dominant limb than for non-dominat limb when running in MRS and CRS (by 12–23%).

Increased reaction time and asymmetrical force generation in muscles may elevate the risk of falling among seniors. Therefore, it seems useful to analyze the symmetry of strength and amplitude parameters that evaluate neuromuscular control. The aim of the study was to evaluate force parameters for the quadriceps and biceps femoris muscles in young and older women performing maximal voluntary contraction. Methods: Fifty women (1 group in their twenties and the other in their sixties) participated in the study. The study used surface electromyography methodology and measured peak torque under static conditions. Electromyographic signals and peak torque were recorded separately in knee extensors and flexors of the right and left lower limbs after a visual signal. The following parameters were selected for analysis: 1) maximum the electromyographic amplitude signal; 2) peak torque; 3) rate of torque development; 4) relative force; and 5) “flexor–extensor” ratio. Results: The analysis demonstrated a decrease in the values of all parameters in the elderly group and symmetry in EMG amplitude in both the younger and older women. Asymmetry was found in the group of elderly women for peak torque and the relative force for knee flexors and “flexor–extensor” ratio. Conclusions: The decline in values of force parameters in knee flexors and their asymmetry (not extensors) revealed in the elderly group might prove an important factor in the assessment of risk factors for falling among the elderly.

The aim of this study was to examine the joint contact forces (JCF) between each limb as the LLD magnitude increases during walking activity. Methods: Eighteen male healthy subjects volunteered to participate in the experiment. Walking gait analysis was conducted with eight different levels of insole to simulate the LLD, starting from 0 cm until 4.0 cm with 0.5 cm increment. Qualisys Track Manager System and C-motion Visual 3D biomechanical tools were used to analyse the results. Four joints (ankle, knee, hip, and pelvis) of lower limb of two legs were investigated. The increment of insoles was placed on the right leg to represent the long leg. Results: The results suggest that the mean contact forces for all joints in the short leg were increased as the increment level increased. On the contrary, the mean contact forces in the long leg decreased when the LLD level increased. Among these four joints, JCF in hip shows a positive increment based on the ASI value. Means that hip shows the most affected joint as the LLD level increase. Conclusions: The result obtained in this study might help clinicians treat patients with a structural LLD for treatment plan including surgical intervention.

This study examined the effects of an on field combined strength/speed/soccer training program on the physical performance of 20 male adolescent (age: 14.0 ± 0.7 years) soccer players, who were divided into an experimental and a control group (EG, CG). The EG players wore "weight shorts", with 300 gr of additional weight on each thigh and participated 2 times/week in a 12-week training intervention program. The CG followed the same training program without wearing the "weight shorts". The physical abilities were evaluated the week before (pre) and two days after the 12-week intervention training period (post) and the follow-up evaluation was performed 4 weeks after the post measurement. The measured parameters were: a) sprint/repeated sprint ability (straight and with 180º turns, RSA-best/mean/total), b) RSA fatigue-index, c) vertical jump ability and d) lower body maximal strength. The EG improved significantly more (p< 0.05) compared to the CG on 10-m-straight, 30-, 35-, 40-m with 180º turns RSAbest-sprints, RSA fatigue-index, squat jump, leg curl (single right/left leg) and in split squat (single right/left leg) 5RM load. Using special shorts with additional weights on the thighs during soccer training improved speed/RSAbest-times and fatigue-index, jump ability and lower limbs maximal strength in youth soccer players.

This study examined the effects of an alternative strength training method during soccer training on the development of physical and technical abilities of youth soccer players. 17 soccer players (age: 15±0.5 years) were divided in a control group (CG) and an experimental group (EG). The players of the EG wore a "weight shorts", with 300 gr of additional weight on each thigh, while they participated into 3 regular soccer training sessions per week, for 12 weeks. The CG followed the same training program without wearing the “weighted shorts”. Before and after 12 weeks of training, anthropometric characteristics, 10- and 30- m speed, the Arrowhead change-of-direction test, the Illinois agility test with ball, the countermovement jump, and the 5-repetition maximum (5RM) load at the split squat, leg extension and leg curl were measured. The EG improved significantly (p< 0.05) more in comparison with the CG in countermovement jump, leg extension and split squat (left leg) 5RM load. The addition of weights on the thighs using special shorts during soccer training sessions leads to increased jumping ability and lower limbs maximal strength in youth soccer players.

Computer simulation methods, based on the biomechanical models of human body and its motion apparatus, are commonly used for the assessment of muscle forces, joint reactions, and some external loads on the human body during its various activities. In this paper a planar musculoskeletal model of human body is presented, followed by its application to the inverse simulation study of a gymnast movement during the take)off from the springboard when performing the handspring somersault vault on the table. Using the kinematic da) ta of the movement, captured from optoelectronic photogrammetry, both the internal loads (muscle forces and joint reactions) in the gym) nast’s lower limbs and the external reactions from the springboard were evaluated. The calculated vertical reactions from the springboard were then compared to the values assessed using the captured board displacements and its measured elastic behaviors.

The aim of this study is to evaluate the influence of the swimming speed during diving on the biomechanical parameters describing the movement of selected measurement points of the lower limb in professional divers. Methods: The study involved a group of 4 professional divers whose movement was recorded during underwater swimming at slow (approx. 0.4 m · s–1), medium (approx. 0.5 m · s–1), and fast (approx. 0.8 m · s–1) pace. Results: During swimming at medium speed, the divers made a smaller displacement (along the axes Y) of the midpoint of fin than during swimming at fast speed. The range of motion in the ankle joint increased in fast speed in comparison with low and medium swimming speed. The same relationship was noted for the obtained velocity and angular acceleration in the hip, knee and ankle joints. The authors observed that during swimming at slow pace the divers choose movement ensuring high swimming comfort while the efficiency of motion is a secondary factor. On the other hand, during swimming at higher pace, the applied movement pattern ensures far greater efficiency rather than swimming comfort. Conclusion: The conducted analysis showed that divers adjust the movement of their lower limbs to the swimming pace.

Despite significant advances in the treatment of diabetic foot ulcers and below-the-knee critical ischemia, there are ongoing efforts to achieve a method with low complication, high success rate and persistence of long-term effects. The aim of the study was to examine the outcome of angioplasty in patients with below-the-knee critical ischemia referred to Hospital. Material and methods. This semi-experimental study conducted on diabetics patients treated with PTA (Percutaneous transluminal angioplasty) with critical ischemia of lower limbs referred to Sina Hospital. After discharge, the patients were followed weekly for the first month and then monthly up to 12 months. The procedure short-term effects were examined through evaluation of wound healing as well as patients' recovery and pain relief, after one month. Given the distribution type, parametric and non-parametric test were used to compare the results before and after treatment. Pearson's correlation coefficient was used to determine the correlation between variables. Results. Twenty four patients participated in this study. The mean ankle-brachial index (ABI) at baseline was 0.55±0.17. A month after angioplasty, the index increased statistically significant to 0.93±0.16. The mean health score expressed by the patients at baseline was 5.48±1.39. A month after angioplasty, it was significantly increased (6.32±1.24). The mean pain score before enrollment was 6.68±2.52 (according to VAS scale). There was a significant decrease over time (3.45±1.13). The overall mean score of all patients at Rutherford Classification was 3.88±0.63 at baseline. During the 1st month and 6th month follow-up, it was changed to Class 0 that was statistically significant in the first month. Conclusions. This study represents the mid-term outcomes of PTA. Although PTA treatment was associated with improved pain scores, satisfaction with health, classification of limb ischemia and diabetic foot ulcers, the effects only remain short-term and mid-term. However, long-term efficacy of PTA needs to be investigated further.

Opis:
Distortion of the axis and shortening of the limbs result in multiple musculoskeletal pathologies. Rotation disorders should also be included among the disorders of the axis of the lower limb. In the case of rotational distortion, only derotation osteotomy can effectively correct torsion-associated deformations. Rotational distortion correction is accompanied by translational displacement and torsion, which results in more complex biomechanics. Using the pedobarographic platform, it is possible to evaluate static and dynamic posture and gait, percentage of body weight distribution on the lower limbs, and balance. Physiological gait and distribution of weight on the lower extremities are symmetrical. Balance is one of the determinants of proper biomechanics of the musculoskeletal system. An important aspect of treatment evaluation is pedobarographic assessment of balance and body weight distribution on the lower extremities ratio. The aim of this work was to evaluate the pedobarographic assessment of body weight distribution on the lower limbs and balance in patients with derotation corticotomies using the Ilizarov method. The study examined a group of 56 patients, who underwent derotation corticotomy using the Illizarov method between 1996 and 2012 at the Clinic of Orthopaedics and Traumatology of the Musculoskeletal System in Wrocław. The control group consisted of 54 patients, who were treated with correctional derotation-free corticotomy using the Ilizarov. Distribution of body weight on the lower limbs and balance were assessed with the pedobarographic platform. Following derotation corticotomy, the amount of body weight placed on the operated limb by subjects from the study group averaged 47.81%, 52.19% in the case of the healthy limb. These differences were not statistically significant. The difference between the average percentage of body weight placed on the diseased and healthy limb in the study group and the controls were not found to be statistically significant. There were no statistical differences in the average length of the gravity line or in the average surface area of the center of gravity position between the study and control groups. Balanced distribution of body weight on the lower limbs was achieved following derotation corticotomies using the Ilizarov method. Derotation corticotomies performed with the Ilizarov method allow for achieving normalization of body weight distribution on the lower limbs and balance, with values similar to those resulting from Ilizarov method derotation-free osteotomy.

Opis:
W pracy przedstawiono propozycję rozwiązania sprzętowego, umożliwiającego ocenę ruchomości kończyn dolnych. Badania przeprowadzono w oparciu o lokalizację istotnych połączeń stawowych kończyny dolnej, po wykonaniu określonego ruchu modelowego. Wykorzystano kontroler KINECT w wersji Kinect for Windows oraz komputer klasy PC i pracowano w oparciu o podstawowe, ogólnodostępne oprogramowanie Kinect SDK.
The paper depicts the hardware solution proposal, which enables the assessment of the lower limbs mobility. The research was founded on the localization of the critical lower limb articulations after fulfillment of the specific model-based movement. KINECT controller in Kinect version for Windows and PC computer were used in the research. The examination relied on basic, widely available Kinect SDK software.
