Malnutrition remains one of the major predictors of mortality in peritoneal dialysis (PD) patients. The aim of the study was to evaluate the nutritional status of prevalent PD patients, and to determine the best predictors of outcome among anthropometric and laboratory indices of nutrition. The study included 106 prevalent PD patients from a single university-based unit. Anthropometric assessment at baseline included: body mass, body mass index (BMI), skinfold thickness, lean body mass (LBM), content of body fat (%F), mid-arm muscle circumference (MAMC). Laboratory analysis comprised of albumin and total cholesterol. Additionally, each patient underwent a subjective global assessment (SGA). The patients were followed for 36 months. Survival analyses were made with the Kaplan-Meier survival curve and the Cox proportional hazard model. Following SGA, malnutrition was diagnosed in 30 (28%) patients. Importantly, eight of the malnourished patients (27%) were nevertheless overweight or obese. Body weight and BMI showed complete lack of association with the outcome. In Kaplan-Meier analysis low: LBM, MAMC, albumin and cholesterol were significantly related to mortality. Cox analysis revealed that, following adjustment, LBM below median was independently associated with poor outcome (hazard ratio [HR] 3.15, 95% confidence interval [CI] 1.17-8.49, p=0.02). Moreover, the lowest quartile of total cholesterol showed independent association with mortality (HR 8.68, CI 2.14-35.21, p<0.01). Malnutrition is prevalent in patients undergoing PD, and overweight/obesity does not preclude its appearance. The most valuable nutritional indices in predicting outcome in this cohort were LBM and total cholesterol concentration.

Abnormal vitamin K status was documented in patients with chronic kidney diseases (CKD), especially those undergoing hemodialysis. The data related to patients undergoing peritoneal dialysis (PD) are contradictory. Therefore, in the present study we aimed to evaluate vitamin K status in patients with CKD who are treated with continuous ambulatory PD. Twenty-eight patients entered into the study. Dialysis vintage ranged from 3 to 89 months. Vitamin K status was assessed in all subjects using undercarboxylated prothrombin measurement (PIVKA-II). In addition, total protein and albumin levels, total cholesterol, LDL cholesterol, triglyceride, calcium, urea and creatinine concentrations were determined. PIVKA-II concentrations were abnormal in 13 (46.4 %) subjects. BMI values, both total and LDL cholesterol concentrations were significantly higher in patients with than those without vitamin K deficiency. Moreover, PIVKA II levels correlated with BMI values (r = 0.441, p < 0.019), LDL cholesterol (r = 0.434, p < 0.021) and creatinine (r = 0.406, p < 0.032) concentrations. However, through the use of logistic regression analysis and multiple regression analysis, no clinical factor was documented to be the independent risk factor of vitamin K deficiency. In conclusion, vitamin K deficiency is a frequent condition in peritoneally dialyzed patients. Assessment of vitamin K status should become a standard procedure in this group of patients.

It is worth looking at chronic illness with its pain, suffering and increasing limitations from the perspective of time. By time we mean subjective time relating to the perception and dynamics of the malady in the patient’s impressions and the role which time plays in the structuring of the patient’s life and experience. The main focus of this article is the experience of progressive kidney failure (uraemia), a condition which requires regular dialysis or kidney transplantation. Due to its specific crises, hopes and periods of waiting, painstaking medical procedures lasting many hours and turning points in the disease’s trajectory, time and its passage are a particularly adequate instrument with which to analyse the experiences of patients with uraemia. These experiences are discussed against the backdrop of selected elements of health care and attitudes towards transplantation which provide the social context for patients’ struggle with illness.

Pacjenci z chorobami nerek są specyficzną, niejednorodną grupą chorych, charakteryzującą się odmienną epidemiologią, wysokim odsetkiem powikłań [2], mającą odmienne rokowania po takich samych procedurach terapeutycznych, jak "populacja nienefrologiczna", wymagającą specjalnego traktowania, innego interpretowania badań biochemicznych, odmienności leczenia [3].

Due to its fluorescent and phototoxic properties, rose bengal (RB) is used in photodynamic therapy. To improve the delivery of RB to its site of action, the application of nanocarrier systems has been proposed. The most promising approach includes the use of pH-responsive nanoparticles. To evaluate the pattern of drug release in different buffers, equilibrium dialysis is commonly used. Here, we used water and two buffers to determine the impact of solvent composition on the aggregation and dialysis rate of RB through a cellulose membrane. The results show that buffer composition does not influence the fluorescent properties of RB. However, the presence of additional ions causes a change in diffusion rate that is most probably linked to the size of RB aggregates.

Introduction: The Peritoneal Dialysis (PD) is a preferable treatment option of the renal replacement in patients with chronic renal failure (CRF) end stage. Purpose: The purpose of this study was to review of articles published related to the contribution of nutrition to health promotion of patients undergoing haemodialysis.Materials and methods: A review of the Greek and international literature on the subject was Performed through the electronic databases Medline, Google Scholar, Scopus and the Association of Greek Academic Libraries Link (Heal-Link), using as key words the following terms:haemodialysis, renal failure, peritoneal dialysis, nutrition.Most of the articles used in this literature review were recently published. Only few old - dated articles were included in the study and the reason was their significant contribution to the field. The exclusion criteria for the articles were the languages except from English and Greek.Results:Patient’s diet must contain 1.3 g of protein per kilogram of body weight per day. Consumption of foods rich in carbohydrates should be limited. Patients should consume foods with the adequate quantity of phosphorus and potassium. Finally, they should have their sodium levels and fluid intake checked because sodium causes severe thirst that can lead to excessive fluid intake. As a result, the patient experiences swelling, shortness of breath and high blood pressure.Conclusion: Patients who undergo peritoneal dialysis must be checked regularly and pay special attention to their diets.

Limb ischaemia caused by formation of dialysis fistula is rare but serious complication. The severity of symptoms may vary but rest pains and necrotic lesions are observed in most advance cases. In these patients different invasive procedures for treatment are performed - from simplest dialysis fistula ligation to complicated vascular reconstructions. The aim of the study was to evaluate treatment results of upper limb ischaemia triggered by dialysis fistula. Material and methods. We have analysed methods and results of treatment of 14 patients with symptomatic upper limb ischaemia caused by dialysis fistula treated in our department between 1st January, 2006 and 30th June, 2013. Treatment was subject to anatomical situation and clinical symptoms. In three patients the ligation of dialysis fistula was performed, four patients underwent inflow reconstruction - in one case by ligation of ligation of vein branch, in three patients by cephalic transfer of arterial anastomosis. In 2 patients hyperkinetic fistula aneurysm was excised and replaced by PTFE bypass, in three patients fistula reconstruction with DRIL method (distal revascularization - interval ligation) was performed, in one patient surgical operation of brachial artery stenosis was conducted. One patient underwent brachial artery angioplasty. Results. Rest pains occurred in all patients (100%), regressive changes in 10 patients (71.4%). Eight patients (57.2%) had concomitant diabetes, seven (50%) ischaemic heart disease, five (35.5%) chronic lower limb ischemia and hyperparathyroidism was observed in fivepatients (35.5%). The imaging studies in all patients revealed pathological steal syndrome (stealing blood to the fistula), in majority concurrent with other pathologies - obstruction stenosis of peripheral artery, defects in blood out flow from the limb. As a result of the surgical treatment, symptoms of limb ischaemia subsided in all patients. Conclusions. Critical limb ischaemia caused by dialysis fistula is a dangerous complication. In most cases there are several causes of ischaemia. Treatment methods should be selected individually for each patient and clinical situation. Clinical symptoms should subside as a result of optimal choice of treatment and, if possible, maintaining of dialysis access.

Introduction and aim. Long-term outcomes of COVID-19 pose a global challenge, particularly impacting individuals with underlying health conditions, including those who have undergone hemodialysis (HD). The study aimed to investigate the relationship between preexisting dialysis dose, measured by single pool Kt/V (spKt/V), and long-term outcomes of COVID-19 in patients undergoing HD. Material and methods. Demographic, clinical, and laboratory parameters following COVID-19 recovery, and long-term outcomes, including the presence of COVID-19 sequelae, hospitalization, and all-cause mortality during a year after COVID-19 were retrospectively analyzed. Results. Out of the 195 patients included, there were 108 males (55.4%) and 87 females (44.6%), with a median age of 56 (44-63) years and a dialysis duration of 49 (31.3–85.2) months. Patients with spKt/V<1.4 had a significantly increased risk of long-term COVID-19 sequelae (HR 9.1, 95% CI: 3.4; 24.6), hospitalization (HR 7.6, 95% CI: 3.9; 14.6), and all-cause mortality (HR 8.5, 95% CI: 2.9; 25.8) within one year after COVID-19 recovery compared with those with spKt/V≥1.4. spKt/V cutoff point of ≤1.3 emerged as a significant risk factor for one-year hospitalization and mortality within our cohort. Conclusion. Suboptimal dialysis dose, as indicated by spKt/V < 1.4, is associated with adverse long-term COVID-19 outcomes in patients undergoing HD. Optimizing dialysis adequacy may mitigate these risks. Further research is needed to validate these findings and explore interventions to improve outcomes in this vulnerable population.

The dialysis of metal ions including Mg(II), Ni(II) and Zn(II) through ultrafiltration membranes made of polysulfone and sulfonated polysulfone was enhanced by sodium salt of poly(acrylic acid) diluted in the receiving compartment. In experiments, concentration of Mg(II), Ni(II) and Zn(II) in the feed solution was equal to 10-3 mol/dm3, concentration of polymer in the receiving solution was equal to 10-2 mol/dm3, and pH = 9. In the presence of complexing polymer, concentration of all metal ions in the receiving solution is higher than 50% for the polysulfone membrane and increases to almost 100% for the sulfonated polysulfone membrane. This effect is due to a cation-exchange property of the sulfonated polysulfone membrane.

Abstract: The objective of this work was to estimate glucose degradation products (GDPs) in solutions for peritoneal dialysis (PD) including glucose and sodium lactate based on the change of pH and absorption in UV. Spectrophotometric and pH-metric methods were used to measure glucose degradation and transformation of GDPs in laboratory-made solutions after heat sterilization and during storage. Mechanism of transformations of GDPs in the sterilized solutions for PD during storage and spectral characteristic of 5-HMF were studied. Common features for all the batches of the tested solutions for PD after heat sterilization were reduced pH, increase in the absorbance in the range 200-350 nm and the appearance of λmax at 274-283 nm. This indicated that 3,4-dideoxyglucosone-3-ene (3,4-DGE) and 5-hydroxymethylfurfural (5-HMF) had been formed during heat sterilization. Hypsochromic shift relative to the 5-HMF was explained by a spectral interference of levulinic acid having λmax at 266 nm. The change of pH after sterilization depended on the initial pH (before sterilization) and glucose concentration. During storage at room temperature hypsochromic or slight batochromic shift was observed. The absorbance at 228-230 nm diminished while that at λmax 270.5-280 nm was slightly reduced or even increased depending on the composition of a solution and time of storage. It was established that 5-HMF had two absorption maxima at the wavelengths of 228-229 and 283-284 nm in water medium and the absorbance of 5-HMF at 284 nm followed Beer’s law very well in the range of concentration of 1.91-9.85 mg/L.

The authors try to explain why the direct, spectra-photometric examination of urea content in dialysate is practically ineffective (in UV-VIS range of spectrum) despite the fact that, in general, spectra-photometry is valuable and very sensitive method of chemical composition determination. An attempt was made to prove that spectra-photometric monitoring of urea dialysis, in chosen parts of UV-VIS spectrum, can be achieved only through indirect measurements.

Aim: Adiponectin and leptin are two adipokines playing important roles in the regulation of body weight, appetite, carbohydrate and lipid metabolism. In patients with chronic kidney disease (CKD) adipokines accumulate in serum due to reduced renal clearance. Since adipokines have pleiotropic actions, the adipokine balance may have an impact on peritoneal membrane transport. The aim of this study was to assess whether serum adiponectin and leptin levels were related to peritoneal transport and residual renal function (RRF) in newly started peritoneal dialysis patients. Methods: 25 clinically stable patients, 4 to 6 weeks after the onset of PD, were included in the study. For each patient clinical and laboratory data were reviewed and standard PET test was performed. Serum adiponectin and leptin concentrations were determined and leptin/adiponectin (L/A) ratio was calculated. Results: Serum adiponectin correlated negatively with weight, BMI and glucose concentration. Serum leptin and L/A ratio correlated positively with BMI. Serum adiponectin correlated positively with dialysate to plasma (D/P) creatinine ratio and ultrafiltration in PET test. Serum leptin level and L/A ratio correlated strongly negatively with peritoneal creatinine clearance. Conclusions: Serum adiponectin concentration is positively associated with baseline solute transport. Leptin concentration and L/A ratio are negatively associated with dialysis adequacy in newly started PD patients.

Background. Dietary supplements (DS) are available over the counter, but patients with impaired renal function are specifically at risk for toxicity when consuming certain DS. The aim of this study was to evaluate the prevalence and characteristics of DS use in patients with chronic kidney disease (CKD). Material and methods. A cross-sectional, controlled DS use survey (22 questions) was conducted among 180 CKD patients (stage 1-5, dialysis, kidney transplant), with 60 patients without CKD serving as controls. Results. DS use did not differ significantly between subjects with and without CKD, unless the CKD patients were on dialysis. In the CKD group, 20% admitted to use DS regularly and 22% did not take the mat all. In the controls, DS consumption was 17% and 13%, respectively (NS). The DS use was higher among women ascompared to men (89% vs. 70%; p < 0.005), and people living in cities versus those living in the country side (81% vs. 63%; p < 0.05). DS most commonly used were: vitamins, minerals, and herbs. Major indications for DS use included: musculoskeletal issues, general health improvement and prevention of urinary tract infections. Subgroup analyses revealed that dialysis patients were characterized by a significantly higher DS use in comparison to CKD stage 1-5 subjects and renal transplant recipients. The decision to introduce DS was made by the physician in 54% of cases; by a pharmacist in 9% of cases, and by the patients themselves in 37%. Only 21% of patients with CKD, and 27% of subjects without CKD, declared knowledge of any possible side-effects associated with DS (NS). Conclusions. The use of DS among patients with CKD is similar to patients without CKD, with the exception of those on dialysis. Vitamins and minerals were the most commonly reported DS consumed. The knowledge on potential side-effectof DS was limited to approximately one-fourth of those surveyed.
