Opis:: We present a patient with intractable and debilitating pain secondary to chronic pancreatitis who was effectively treated with total pancreatectomy with islet autotransplantation (TPIAT). Islets engrafted into his liver significantly contributed to improved blood glucose control and quality of life. Subsequently, the patient developed alcohol related acute liver failure and en bloc liver and pancreas transplantation was performed to replace the failing liver with engrafted islets. Pancreas transplantation was required to resolve his life-threatening severe hypoglycemic episodes. Herein, we detail an innovative and multidisciplinary management of this complex medical problem.

Opis:: Background: Fibrinogen is a protein playing pleiotropic role in human body. It is engaged in maintaining hemostasis. Congenital fibrinogen disorders comprise quantitative and qualitative fibrinogen anomalies. The symptoms range from bleeding, thrombosis to asymptomatic at all what is the most common case. Hypodysfibrinogenemia with lower level of fibrinogen of reduced activity, is the least common of all congenital fibrinogen disorders. Case report: A 31-year-old woman was reported at the 21 weeks of gestation, suffered from genital tract bleeding and there was a history of stillbirth. Clinical examination with no pathology, however laboratory tests revealed coagulation abnormalities due to prolonged thrombin test, decreased protein S and lower fibrinogen level (70 mg/dl). Autoimmune diseases were excluded and the diagnosis was widened with rotational thromboelastometry and genetic test for hypodysfibrinogenemia. The patient was treated with fibrinogen substitution and prophylactic dose of heparin throughout pregnancy and 2 weeks following labour. At 39 week of gestation Caesarean section was done, with no complications. Results: Genetic test revealed heterozygous mutation in fibrinogen gamma gene confirming hypodysfibrinogenemia. Due to bleeding manifestation in this patient of congenital fibrinogen disorders, fibrinogen substitution was implemented with heparin as a paranticoagulant prophylaxis, what turned out to be successful and enabled the patient to maintain the pregnancy. Conclusions: As hypodysfibrinogenemia symptoms are diverse the management is difficult and each patient’s therapy should be planned separately. Pregnancy may be the first time when congenital fibrinogen disorders reveal and it is especially challenging to prevent from obstetrical complications.

Opis:: Fieldwork in early 2019 by the Gebelein Archaeological Project encompassed surveys of two cemeteries situated south of the ancient town of Per-Hathor/Pathyris in the area of the Eastern Mountain of Gebelein. One of these is dated to the Old Kingdom and the First Intermediate Period, the other tentatively to Fatimid times. The third survey searched for local chert sources on the Western Mountain, investigating a local tradition of lithic tool production.

Opis:: Continued archaeological surveys at two sites in the Gebelein area, the Northern Necropolis and the temple complex, have contributed new data for a better understanding of the ancient remains. Geophysical anomalies detected in 2015 in the western part of the Northern necropolis should now be interpreted most probably as tombs with mud-brick walls. Mounds of earth in the central part of the necropolis yielded numerous artifacts dating from between the Naqada I and the early Old Kingdom periods; they are likely to have been dumped from a nearby settlement site, probably the ancient town of Sumenu. Work in the temple complex was aimed at protecting the structure made of inscribed mud-bricks dating from the Twenty-first Dynasty.

Opis:: All periods of Egyptian history are represented at Gebelein and it encompasses many kinds of archaeological site found in the Nile valley (that is, cemeteries, settlements, fortifications, temples, rock quarries etc.). The area was a significant centre in the history of ancient Egypt, but its exact role and the reasons for its importance still awaits explanation. That is why the Gebelein Archaeological Project was initiated. The field survey of Gebelein, which started in 2014, aims to recognise the topography of the area and the degree of destruction of the sites and to locate archaeological remains as well as to determine their interpretation and dating. This report outlines the results of work conducted during seasons 2014, 2015 and 2016, which encompassed the archaeological and epigraphic field surveys, geophysical prospection as well as work conducted in the rock-cut chapel of Hathor.

Opis:: The use of bone cement in procedures such as vertebroplasty and kyphoplasty can reduce pain and mechanically support the spine. This study aimed to evaluate whether air entrapped within bone cement affected its distribution in a vertebral body model. The study included 3D printed anatomical models of vertebrae together with their internal trabecular structure. Aeration was achieved by mixing the bone cement using three different altered procedures, whilst the control sample was prepared according to the manufacturer’s instructions. The further two samples were prepared by reducing or increasing the number of cycles required to mix the bone cement. A test rig was used to administer the prepared bone cement and introduce it into the vertebral model. Each time the injection was stopped when either the cement started to flow out of the vertebral model or when the entire cement volume was administered. The computer tomography (CT) scanning was performed to evaluate aerification and its influence on the bone cement distribution in each of the patient-specific models. The large number of small pores visible within the cement, especially in the cannula vicinity, indicated that the cement should not be treated as a homogenous liquid. These results suggest that a high level of aerification can influence the further cement distribution.

Opis:: W niniejszej pracy przedstawiono kolejną wersję modelu dla prognozowania podstawowych wskaźników makroekonomicznych z wykorzystaniem danych z testów koniunktury. W pracach Białowolskiego, Kuszewskiego i Witkowskiego (2010a, 2010b, 2011, 2012a, 2012b) rozwijano metodykę budowy modeli dla prognozowania tempa zmian produktu krajowego brutto, stopy bezrobocia i wskaźnika cen towarów konsumpcyjnych. W zbiorze regresorów tych modeli, oprócz opóźnionych w czasie zmiennych endogenicznych, uwzględnia się wyłącznie wyniki różnych testów koniunktury. Badanie dotyczy specyfikacji modelu prognostycznego metodą bayesowskiego uśredniania klasycznych oszacowań (Bayesian averaging of classical estimates, BACE). Przyjęte rozwiązanie umożliwia automatyzację proces doboru postaci modelu. W kolejnym etapie postępowania jest rozważany wpływ sezonowości deterministycznej i stochastycznej szeregów czasowych na wynik procesu prognozowania. Zaproponowano intuicyjną procedurę uwzględniania obu rodzajów sezonowości w procesie prognozowania. Po zakończeniu procesu estymacji i doboru modeli weryfikowano ich możliwości prognostyczne.
This paper presents another version of model designed to forecast main macroeconomic indicators with the use of economic survey data. In previous papers (Białowolski, Kuszewski, Witkowski, 2010a, 2010b, 2011, 2012a, 2012b) methods for developing models used for forecasting GDP growth rate, unemployment rate and CPI were proposed. The set of regressors in those models included only lagged dependent variables and indices based on various survey data. In this paper the specification of the forecasting model is selected with the use of Bayesian averaging of classical estimates (BACE). This algorithm enables an automatic process of selection of functional form of the model. Next the influence of deterministic and stochastic seasonality in time series on forecasting process is concerned. An intuitive procedure of applying and selecting among both types of seasonality in the forecasting process is discussed. Afterwards their forecasting capabilities are considered.
